Analysis 

The pilot reported that, during preflight preparations, he estimated that the flight time would be about 2 hours 29 minutes with a total fuel burn of 10.1 gallons. The airplane departed with 12 gallons of fuel. About 2 hours 25 minutes into the flight, the engine quit. The pilot navigated toward a nearby field to make an off-airport landing. During the landing, he saw a deep ditch and a house, so he attempted to ground loop the airplane to the left. Both wings impacted trees. The airplane sustained substantial damage to both wings. The pilot reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation. The pilot reported as a safety recommendation that adding a fuel stop would have prevented the accident.

The Federal Aviation Administrator inspector who examined the airplane reported that the nose tank had evidence of fuel being in it recently (the indicator cork was still wet with fuel), but no fuel remained in the tank. Both the right and left wing tanks were examined, and no fuel remained in either tank. No evidence of a fuel spill was found on the airplane or the ground.

Analysis 

The pilot reported that, during the takeoff roll, the gyroplane's rotor "flapped" and struck the rudder. He aborted the takeoff and taxied to the ramp without further incident. The gyroplane sustained substantial damage to the rudder. The pilot reported that there were no preaccident mechanical failures or malfunctions with the gyroplane that would have precluded normal operation. The pilot stated that if he had aborted the takeoff at the first indication of a problem the accident could have been avoided.

Analysis

The pilot reported that he performed three low passes and briefly touched down on the 750-ft-long grass runway to test the surface before conducting a full-stop landing. The pilot stated that, while landing to the northeast, the airplane bounced and veered right, and he attempted to correct using left rudder. During the landing roll, he heard a noise that sounded like branches brushing against the right side of the airplane and decided to go around by applying full power and retracting the flaps. Concerned that the airplane would not clear the large bush at the end of the runway, he reduced the power to idle and used left rudder to steer the airplane left of the bushes. The airplane impacted the bushes, and the left main landing gear collapsed. The airplane sustained substantial damage to both wing lift struts and the elevator. The pilot reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation.

Analysis 

The flight instructor reported that, while en route on an instrument flight plan, about 4000 ft, the airplane entered a cloud where a flock of large black birds were circling. A bird impacted the stabilator, and he then disengaged the autopilot and checked the flight controls for any adverse effects; he did not see any. The instructor then chose to return to the departure airport and landed without further incident. The airplane sustained substantial damage to the stabilator. On October 18, 2019, at 1430 eastern daylight time, an Ayres S2R-G10, N762BF, was destroyed by fire after a precautionary landing in a pasture in Shellman, Georgia. The commercial pilot was not injured. Day visual meteorological conditions prevailed, and no flight plan was filed for the aerial application flight, which was conducted under the provisions of Title 14 Code of Federal Regulations Part 137. The flight departed a private field in Shellman, Georgia about 1330, and was destined for Andrews airport (4GA8), Arlington, Georgia.

According to the pilot, after completing an aerial application, he was heading to his home base at 4GA8. While at an altitude of 600 ft mean sea level, the engine began "surging as if it were starving for fuel." He had fueled the airplane with 75 gallons prior to departure. He decided to perform a precautionary landing in a pasture. During the rollout, he noticed the airplane was headed toward an irrigation center-pivot sprinkler and decided to ground loop the airplane to avoid hitting it. During the ground loop, the right main landing gear and tail wheel assemblies collapsed. The pilot reported that he remained in the airplane for "a few seconds" before exiting, and when he did, he noticed smoke coming from the engine nacelle. He then called the landowner and informed him of the off-field landing and advised that the airplane was burning and asked the landowner to notify the fire department.

An examination of the wreckage at the accident site by a Federal Aviation Administration (FAA) inspector revealed that the airplane was mostly consumed by fire. The source of the fire could not be determined at the scene. The airplane was recovered to a salvage facility and retained for further examination.

The pilot reported that the airplane's most recent 100-hour inspection was performed on August 21, 2019 at which time it had accrued a total of 1,788 flight hours.

According to FAA airmen records, the pilot held a commercial pilot certificate with a rating for airplane single-engine land. The pilot reported 20,000 hours of total flight experience, with 19,500 hours in the same make and model as the accident airplane.

On October 18, 2019, about 1530 eastern daylight time, a Beech N35 airplane, N564T, impacted terrain and trees during a go-around at Monroe County Airport (MNV), Madisonville, Tennessee. The private pilot was not injured. The airplane was operated by the pilot as a Title 14 Code of Federal Regulations Part 91 personal flight.

The pilot reported that during the approach to MNV he decided to perform a training exercise by simulating an engine failure during the landing. He entered the traffic pattern for runway 5 and reduced power to idle; while on final approach he reported that the airplane was "a little fast and a little high" so he performed a forward slip to the left (left rudder application) for a "second or two." He continued the approach and the airplane touched down with about 1,000 ft of the 3,643 ft runway remaining and immediately bounced back into the air.

The pilot decided to perform a go-around and applied power; however, the engine did not respond. The airplane flew beyond the departure end of the runway, touched down in grass, and skidded into shrubs and trees. The left and right wings sustained substantial damage. Postaccident, the pilot reported that he believed the slip maneuver "unported fuel from the right tank [selected tank for approach] and prevented the plane from throttling up." He reported that there were no mechanical malfunctions or failures with the airplane.

Examination of the airplane by a Federal Aviation Administration inspector found no preimpact mechanical malfunctions or failures that would have precluded normal operation. There were no obstructions or debris found in the fuel system and the engine produced normal power when it was test run. Inspection revealed 17 gallons of fuel in the right tank and 22 gallons in the left tank.

The airplane flight manual stated in part:

FUEL CELLS

Fuel supply is carried in two bladder-type cells with a total capacity of [40 gallons, optional fuel system installed], located in the wings just outboard of the fuselage. Fuel is fed from the cells to a selector valve just forward of the front seat, on the left side, then through a strainer to the fuel pump and the engine. The fuel tank fillers are located in the wing leading edges.

Maximum Slip Duration:

30 seconds for airplanes with baffled main fuel cells in both wings.
