Flight Conducted Under: Part 91: General aviation - Personal

On April 18, 2022, about 1329 central daylight time, a Piper PA-28-140B, N8891N, was substantially damaged when it was involved in an accident near Panama City, Florida. The pilot and passenger were not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

According to a Federal Aviation Administration (FAA) inspector, the airplane had not flown “in years” and was stored outdoors at Sandy Creek Airport (75FL), Panama City, Florida. The pilot reported that the airplane had been started and taxied during that time and he had been briefed about the condition and history of the airplane by the owner and an airframe and powerplant mechanic who had completed an annual inspection on the airplane about 2 months prior to the accident. On the day of the accident, the pilot planned to take a short flight in the general vicinity of 75FL Prior to the flight, the pilot checked the fuel and oil quantity. No water or debris was observed in the fuel sampled from the drain ports and about 25 gallons of fuel was on board. Ground operations were unremarkable. The engine ran smooth, and all indications were within limits.

Prior to takeoff, the pilot decided to perform the pre-takeoff checks twice, each followed by a highspeed taxi down the runway to rotation speed, and “The aircraft performed flawlessly.” The pilot then performed a final engine runup and magneto check, lined-up on the runway, advanced to full throttle, and began the takeoff roll. The airplane accelerated and became airborne as expected at 60-65 mph with only slight back pressure. The initial climb rate was 300-500 feet-per-minute and the airplane accelerated to 85 mph, best climb rate.

The pilot continued to allow the airplane to accelerate towards 100 mph for en route climb and engine cooling, but as the airplane approached the departure end of the runway, the engine rpm suddenly decreased to about 2400 rpm, and airspeed began to decrease. The pilot ensured the mixture was full rich and the throttle was full in; however, the engine rpm subsequently continued to decrease as if he was slowly pulling the throttle to idle.

The pilot attempted to perform a forced landing to road; however, the airplane was unable reach the road and struck trees before it impacted the ground.

Examination of the airplane by an FAA inspector revealed that during the impact sequence, the right wing, and right horizontal stabilator had separated from the airplane. The engine revealed that water was present in the boost pump and carburetor float bowl. Corrosion was present in the engine driven fuel pump, and the carburetor inlet screen displayed several areas where contamination and debris were adhering to the screen.

The wreckage was retained for further examination.

Location: Tuskegee, Alabama
Accident Number: ERA22LA197
Date and Time: April 15, 2022, 14:30 UTC

Aircraft: Cessna 182R
Aircraft Damage: Substantial
Defining Event: Hard landing
Injuries: 1 None
Flight Conducted Under: Part 91: General aviation - Personal

Analysis

The pilot reported that while landing and during the landing flare, a gust of wind caused the airplane to drift off centerline and the nose to rise. The airplane ballooned, then subsequently bounced hard before ballooning a second time. The nose dropped sharply, and the pilot was unable to control the airplane before it struck the runway hard on the nose landing gear, causing the landing gear to collapse and fold back, which resulted in substantial damage to the engine compartment, firewall and underside of the airframe. The pilot stated that there were no pre accident mechanical failures or malfunctions with the airplane that would have precluded normal operation.

Probable Cause and Findings

The National Transportation Safety Board determines the probable cause(s) of this accident to be:
The pilot’s inadequate compensation for the gusty crosswind wind conditions which resulted in a hard landing and subsequent nose gear collapse.

Findings

Aircraft Directional control - Not attained/maintained
Personnel issues Aircraft control - Pilot
Environmental issues Gusts - Response/compensation
Environmental issues Crosswind - Response/compensation

On April 15, 2022, about 0530 mountain standard time, a Piper PA-24-400 airplane, N8493P, was substantially damaged when it was involved in an accident near Benson, Arizona. The pilot was not injured. The airplane was as operated as a Title 14 Code of Federal Regulations (CFR) Part 91 personal flight.

According to the pilot he was approaching Benson Municipal Airport (E95) to land when a departing helicopter reported there were deer near the runway. The pilot said he “turned on the landing lights, dropped the gear and started my approach to runway 10.” He did not see any animal activity and proceeded to land. Upon landing the airplane began to shake and the airplane stopped on the runway. The airplane came to rest on the belly of the airplane and the main landing gear were observed partially extended. The nose gear was not visible due to the airplane position on the runway.

According to personnel who recovered the airplane, the landing gear switch was in the down position when the airplane was recovered. When lifted the landing gear remained in a partially extended position and the landing gear did not operate when power was applied to the airplane and the landing gear switch was manipulated. The landing gear subsequently lowered and locked when the emergency gear system was utilized. The airplane received substantial damage to the skins, frames, and stringers on the underbelly of the fuselage.

Analysis

The flight instructor and student pilot were practicing a tow rope failure and were landing the glider with a light, quartering tailwind. On final approach the glider drifted right of the runway centerline and just before touchdown, the right wing struck a tree, which resulted in substantial damage to the right wing and canopy. The flight instructor reported that the airplane had no preimpact mechanical malfunctions or failures that would have precluded normal operation.

Probable Cause and Findings

The National Transportation Safety Board determines the probable cause(s) of this accident to be:
The flight instructor’s failure to maintain directional control with a quartering tailwind.

Findings

Personnel issues Aircraft control - Instructor/check pilot
Aircraft Lateral/bank control - Not attained/maintained
Environmental issues Tailwind - Response/compensation
Environmental issues Tailwind - Awareness of condition

BUNN, North Carolina (WNCN) — A glider crashed near an airport in Franklin County Saturday afternoon, officials said.

The incident was reported around 3:30 p.m. near Crooked Creek Airport, which is about 4 miles north of Zebulon, according to a news release from the Federal Aviation Administration.

The glider involved was a Schweizer SGS 2-33A and only the person piloting the glider was onboard at the time, the news release said.

Frank Swett, the property owner where the glider crashed, said a student pilot was inside the glider but no one was injured.

The two-seat glider, with a registration number of N7784S, was built in 1969 and is registered to the North Carolina Soaring Association.

The North Carolina Soaring Association appears to be based at Crooked Creek Airport, according to the group’s website.

Swett said the foundation teaches teens and young adults how to fly gliders.

A single-engine plane crashed and flipped at the Richland Airport on Sunday afternoon.

The crash happened just before 4 p.m. at the airport operated by the Port of Benton off the Highway 240 bypass.

Port officials at first were unable to spot the overturned plane in the brush along the runway, according to initial dispatch reports.

The pilot reportedly called 911 for help from inside the plane.

It’s unclear if more than one person was onboard, however, there were reports that at least one person was injured.

No information was immediately available from Richland police on Sunday.

The airport serves regional business customers and has two paved runways, according to the port’s website.
