Analysis

The pilot reported that after a stabilized approach to land, during the touchdown a gust of wind caused the left wing to raise and go "vertical" or "knife edge." He immediately applied full power to go-around, however, the airplane was banking and drifting to the right and before the airplane could climb, it impacted terrain to the right of the runway and skidded down a ravine. The left and right wings sustained substantial damage. The pilot reported that there were no preimpact mechanical failures or malfunctions with the airplane.

About the time of the accident the weather observation at the airport included wind from a heading of 320° at 9 knots, gusting to 18 knots. The calculated crosswind component for the landing on runway 5 was 14 knots.

Analysis

After an uneventful local flight in the single-engine airplane, the pilot entered the landing pattern at his destination airport and the windsock indicated an almost perpendicular crosswind of about 10 knots. On final approach, after clearing a tree line, the pilot reduced engine power to land. As engine power and airspeed were reduced, the airplane's rate of descent suddenly increased. The pilot attempted to slow the rate of descent, but the airplane impacted the runway hard and bounced. The airplane exited the runway and impacted a berm. The nose landing gear collapsed, resulting in substantial damage to the front lower part of the fuselage and firewall. The pilot stated that, due to the crosswind and the trees at the end and side of the runway, he should have adjusted his landing point further down the runway to avoid possible wind anomalies.

Factual Information

On June 7, 2020, about 1513 eastern daylight time, a Team Mini-Max 1600R airplane, N827WS, was substantially damaged when it was involved in an accident near Marshall, Michigan. The pilot was not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

According to the pilot, the engine lost total power during initial climb after departing from a campground area. The pilot made a forced landing into a cornfield, which damaged both wing struts.

During the flight prior to the accident, the pilot noticed engine roughness that discontinued after he turned the fuel pump on. The pilot inspected the airplane after landing but was not able to find a rationale for the engine roughness.

Following the accident, examination of the engine revealed that the fuel supply line to the inlet fitting of the vacuum driven fuel pump was loose on the fitting. Although the clamp on the inlet fitting had been tightened to its maximum clamping position, the fuel supply line was still loose on the fitting and without a proper seal it was possible for air to enter the fuel system.

Analysis

The flight instructor stated that he was acting as a safety pilot for the newly certificated pilot, who was getting familiar with the airplane. They had flow five or six circuits around the traffic pattern and subsequently landed on runway 13. When the airplane was just above taxi speed, it began drifting to the right. The pilot used full left rudder and "stomped on the brake." The airplane veered to the left and then the right wingtip contacted the ground. The outboard one-quarter of the right wing was crumpled and the right main landing gear collapsed under the fuselage. The flight instructor stated that there were no mechanical malfunctions or anomalies with the airplane that would have precluded normal operation.

On June 8, 2020, about 0945 Alaska daylight time, a float-equipped Piper PA-12 airplane, N3188M, sustained substantial damage when it's rudder structurally failed in flight about 8 miles north of Anchorage, Alaska. The flight instructor and private pilot receiving instruction were not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 instructional flight.

According to the flight instructor, they departed Lake Hood Airport (PALH) and proceeded to Twin Island Lake (about 8 nm northwest of PALH), where upon arrival, they conducted a normal landing. After departing, they climbed to about 500 ft above ground level (agl) before turning a left crosswind traffic pattern leg. While on the left crosswind leg, the airplane yawed abruptly to the right and the private pilot indicated that the controls felt strange. The flight instructor assumed control of the airplane and noticed drastically diminished control about the vertical axis. In addition, a significant downward elevator pressure (forward control yoke) was required. In an effort to aid in directional control the water rudders were deployed. Uncertain that he could make a 180° turn and return to Twin Island Lake due to the poor directional control, he elected to return to PALH where emergency services were available and conducted an uneventful landing.

An initial examination of the rudder revealed that the vertical spar tube separated just above the upper hinge point and the top portion of the rudder folded over the horizontal stabilizer tail brace wires.

The rudder has been retained for further examination by the National Transportation Safety Board (NTSB) Materials Laboratory.

Analysis

The pilot reported that after about 15 minutes of flying around the airport, he returned for landing. While he was on the base leg of the airport traffic pattern, he thought the airplane was a little high, so he reduced engine power so he could join the final approach on a 3-degree glide slope. As he entered the final approach, he added a little power and aimed for a touchdown point about 1,000 ft down the runway, which was sloped; it was higher on the approach end and dipped down in the middle. During short final, he realized that he descended below the glide path and his sink rate was higher than expected, so he added engine power, but shortly after doing so, he felt a "bang" and jolt, followed by a pitch down and impact with the terrain. The airplane struck a localizer antenna near the approach end of the runway and skidded to a stop in the grass. The forward portion of the fuselage, right wing, and horizontal stabilizer were substantially damaged. The pilot reported that there were no preaccident mechanical malfunctions or failures of the airplane that would have precluded normal operation.

Analysis

The solo student pilot reported that during landing, the airplane landed hard. The pilot subsequently taxied the airplane to the ramp without further incident. The forward fuselage, aft of the firewall was substantially damaged.

The pilot reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation.

Prior to the aerial application flight, the airplane was loaded with about 3,400 pounds (lbs) of dry fertilizer and contained about 140 gallons of fuel. The pilot's estimated weight at the time of the accident was 9,000 lbs, with a maximum gross weight of 9,400 lbs. The pilot reported the temperature was about 95° F, the wind was from 150° about 6 knots, and runway 35 was rough with tall grass. The calculated density altitude was about 3,900 ft. During the takeoff, the pilot attempted to takeoff; however, "the aircraft was not ready to fly." The airplane impacted a field about 500 ft north of the runway. The airplane sustained substantial damage to the forward fuselage. The pilot reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation. The pilot's recommendation to prevent this accident was "carry lighter loads, less fuel on hot days."

DESCRIPTION OF ACCIDENT:  
On June 8,2020 @ Hiawatha Municipal Airport (K87) in Hiawatha, Kansas, encountered heavy, downwind takeoff on a hot day with tall grass turf. Pilot remained in ground effect at the end of the runway, not gaining altitude and struck the ground with the main gear, went through a fence, struck two terraces and crashed the aircraft nose first into a newly planted soybean field.

Aircraft dismantled, including cutting of tubes and wires and structures and relocated to Dodson International in Rantoul, KS.
