Analysis 

According to the flight crew, the instrument approach was flown above the glideslope in anticipation of mechanical turbulence. As the airplane descended through 500 ft, the pilot monitoring notified the pilot flying that the airplane was high on the approach. The airplane continued the approach above the glideslope, and the landing flare was conducted too high, which resulted in an excessive sink rate. The pilot flying attempted to arrest the sink rate but was unable to do so, and the airplane landed harder than expected, which resulted in a tail strike and substantial damage to the lower aft fuselage.

The pilot in the tailwheel-equipped airplane reported that during landing, the airplane encountered a variable crosswind gust and the left wing lifted. He was able to keep the airplane on the runway and established a landing roll. The ground speed was about 20 miles per hour and the airplane encountered another wind gust. The airplane weathervaned to the left and exited the left side of the runway, rolled into mud and nosed over.

The airplane sustained substantial damage to the rudder.

The pilot reported that the wind at the time of the accident was variable at 5 knots and gusting to 8 knots.

The METAR at the nearest airport was about 12 miles to the west of the accident site, and reported that about the time of the accident, the wind was out from 02 at 6 knots. The pilot landed the airplane on runway 25.

The pilot reported that there were no mechanical malfunctions or failures with the airplane that would have precluded normal operation.

Her airplane then continued to the right, turning about 180° with her right wing striking the propeller of the high-wing airplane, followed by her propeller striking the right wing of the high-wing airplane.


The flight instructor in the high-wing airplane reported that after exiting the runway on to a taxiway he "felt a small hit on the left side." He saw a low-wing airplane make a right 180° turn around the front of his airplane and impact the right wing of his airplane.


The low-wing airplane sustained substantial damage to the right wing. The high-wing airplane sustained substantial damage to the right wing.


The student pilot reported that there were no mechanical malfunctions or failures with the airplane that would have precluded normal operation.

Analysis 

The pilot in the retractable-landing gear-equipped airplane reported that, during an instrument flight rules night flight, he initiated an approach in instrument meteorological conditions. During the approach, he saw that rime ice had accumulated on the left wing. He continued the approach, extended the landing gear, and then configured the flaps for landing.

The pilot attempted to level off at 1,620 ft, although the minimum descent altitude was 1,600 ft, but the airplane stalled. He applied forward pressure on the yoke to lower the nose and applied full throttle. He recalled that the airplane was extremely slow to respond and that it was "flying on the ragged edge of stall." The airplane exited the clouds, and he could see the airport, but the airplane impacted a field about 2.4 nautical miles short of the runway.

The airplane sustained substantial damage to both wings and the fuselage.

The pilot reported that there were no mechanical malfunctions or failures with the airplane that would have precluded normal operation.

On December 6, 2018, about 0905 Pacific standard time, Southwest Airlines flight 278, a Boeing 737-7H4, N752SW, overran runway 08 during the landing roll at Bob Hope Airport (KBUR), Burbank, California, and came to rest in the engineered materials arresting system (EMAS) at the departure end of the runway. None of the 117 passengers and crew onboard were injured and the airplane sustained minor damage. The regularly scheduled passenger flight was operating under the provisions of 14 Code of Federal Regulations from Metropolitan Oakland International Airport (KOAK), Oakland, California to KBUR.

The pilot reported that following his first solo flight in the recently acquired airplane, during landing he crossed the runway threshold about 70 knots airspeed and with full flaps, he placed the throttle in the idle position. During the flare, the airplane sank abruptly and touched down hard. The airplane bounced and exited the left side of the runway. The pilot attempted to go-around, but the airplane descended and impacted the grass safety area on the left side of the runway. The nose landing gear separated from the airplane.

The airplane sustained substantial damage to right fuselage formers.

The pilot reported that there were no mechanical malfunctions or failures with the airplane that would have precluded normal operation.

The pilot in the tailwheel-equipped airplane reported that he had performed several takeoffs and landings from the shoreline of a lake. He reported that during the takeoff roll about 25 knots airspeed, a crosswind gust from the right, pushed the airplane to the left and into the water. The left main landing gear entered the water and spun the airplane 90° to the left, and the airplane nosed over.

The airplane sustained substantial damage to the right wing and rudder.

The pilot reported that during takeoff the wind was out of 180° at 20 knots and gusting to 45 knots.

The METAR for an airport located 4 nautical miles from the accident site reported that about the time of the accident, the wind was out of 210° at 11 knots. No gusts were reported. The pilot reported that his direction of takeoff was to the southeast.

The pilot reported that there were no mechanical malfunctions or failures with the airplane that would have precluded normal operation.
