On August 26, 2021, at 1352 eastern daylight time, N413N, an Embraer E55P (Phenom 300), sustained substantial damage when it collided with a parked vehicle on landing at the Elk River Airport (NC06), Banner Elk, North Carolina. The two-airline transport rated pilots were not injured. The airplane was operated as a Title 14 Code of Federal Regulations (CFR) Part 91.

According to the Elk River website, “The Elk River Airport is a private facility for the exclusive use of the Elk River Property Owners Association members and their approved guests and associate members of the Elk River Club.” All pilots are advised to “…carefully read the Elk River Pilot Manual (pdf) and also view the following video presentation of arrival and departure procedures for the Elk River Airport.” The airport is restricted to daytime, visual flight rules (VFR) operations only, and there is a published VFR approach chart available on their website.

According to the captain, he and the first officer planned for the flight into NC06 the day before the accident. They reviewed forecasted weather and landing performance data for landing on both a dry and wet runway. They also reviewed the airport video and manual. The captain said as they were enroute to NC06 the following day, he and the first officer observed a small rain cell in the vicinity of the airport. They briefed that if they could not make the visual approach they would divert to another airport or hold until the weather passed.

However, as they approached the airport, the captain made visual contact with the runway and began the visual approach into the airport using the visual cues recommended in the airport video. The airport was reporting calm winds and light rain. The captain said he flew the entire recommended VFR approach and was fully configured to land and slowing to Vref speed when they were on a 1-mile final. They also capture the visual approach slope indicator (VASI) lights and proceeded to land. The captain said, “As soon we touched down I applied full brakes only to find the brakes were not slowing the airplane, by the time we realized the plane was not slowing it started to drift right and went off the runway slightly on the right side of the runway … I see the cliff off the end of the runway and apply full left rudder and steering to the left to try and steer toward extra terrain on the left to avoid going of the cliff, as we go thru the small taxiway and into the other grass area, we took a sign with our wing. As we are still moving forward, we hit a beige Jeep on our wing tip which actually caused us to stop and saved our lives by not allowing us to go off the cliff 5 feet in front of us.”

Postaccident examination of the airplane revealed substantial damage to the left wing.

The airplane was equipped with a combination cockpit voice and flight data recorder, which was retained for further examination. The airplanes braking control unit (BCU) was also retained.

A small plane went down in the water just off Edmonds’ Marina Beach around 4:30 p.m. Thursday afternoon. The pilot, who was rescued by a private boat, was uninjured and there were no passengers on board, Edmonds police said.

Shortly after the pilot  — a 50-year-old man from Buckley, Washington — was rescued, the plane began to sink. It is now completely submerged, authorities at the scene said.

Edmonds resident Adam Bock is the boater who rescued the pilot.

Bock, who works at Boeing, was in a boat trolling about half a mile in front of the Edmonds marina breakwater when the plane went over the top of him.

“I knew he was in trouble,” Bock said. “I saw it go over and the prop wasn’t spinning, and he was really low, and I was just trying to get the fishing gear up as fast as I could.”

Bock said the pilot was flying north with a couple of friends in other planes when the engine apparently quit, Bock saw him turn back toward the beach to try to set it down there, but there were too many people, so the pilot “just flared it out and made an amazing landing.”

The pilot set the plane down about 100 feet from the beach, Bock said. “I had the boat moving as he hit the water…had my radio on, called the Coast Guard.”

When the boat came up alongside the plane, the pilot was standing on the wing.

“He was standing on the wing, just waving me down… had just a little water on his vest… and stepped onto the swim step of the boat, and I asked, ‘are you OK’?” Bock explained. “Another minute and he’d have been swimming for the beach.”

The two tried to get a rope around the plane, but in less than two minutes, it sank into the choppy water, with waves about 2 feet high.

Bock described the pilot as “super calm, he wasn’t shaking, there was no panic.” He then dropped the pilot off at the Edmonds Marina fuel dock, where police, fire and Coast Guard officials interviewed both men.

When a firefighter called him a hero, Bock replied that “it wasn’t a big deal, I just got the guy out.”

Bock decided to go back out and try fishing again, thinking that the rescue may have brought him good luck. Nope, the fish didn’t play along.
 
Edmonds police spokesperson and Acting Assistant Chief Josh McClure said that the pilot reported there was a mechanical failure. Police were working to notify the Federal Aviation Administration and the National Transportation Safety Board about the crash. Although the pilot was uninjured, he was “understandably pretty shaken,” said McClure.

On August 25, 2021 at 0945 Pacific daylight time, a Cessna 150D, N4375U, was substantially damaged when it was involved in an accident near Ontario, California. The pilot sustained minor injuries. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

The pilot stated that prior to departure she added 16 gallons of fuel, which brought the total fuel onboard to 28 gallons. The pilot maneuvered in the local area until about for about 2 hours and 45 minutes before returning back to the airport. On the descent to 2,500 feet mean sea level (msl), the engine began to surge and in response the pilot added carburetor heat. The engine sounded normal for about 3 to 4 minutes and then began to surge again. As the airplane approached the airport the pilot noticed that the engine experienced a partial loss of power and was running around 1,000-1,200 rpm. The pilot attempted to troubleshoot the loss of power by verifying the mixture was rich, the fuel selector was positioned to “BOTH,” and the and the magnetos were both selected.

The pilot further stated that despite her attempts, the airplane could not sustain level altitude and she selected an area to perform an off-airport landing. About 20 seconds prior to touchdown the engine experienced a total loss of power and the airplane collided with a fence. As a result of the impact, the airplane sustained damage to the wings.

The Federal Aviation Administration inspector stated that he arrived at the accident site that afternoon. He stated that the fuel tanks were intact and he only observed a small drip from the left fuel vent. The owner stated that he recovered the airplane. During the recovery, there was about 3.5 to 4 gallons drained from the left wing and about 0.5 gallons from the right wing. He added that the carburetor bowl and gascolator both contained fuel.

The pilot of a Cessna 150D suffered minor injuries Wednesday morning after she forced her single-engine plane onto a vacated dairy property in Ontario after experiencing engine trouble while trying to land at Chino Airport, Ontario Police said.

The pilot’s name, age and city of residence was not released, but she was taken to a hospital for treatment, said Ontario Police Officer Sequoia Payton.

“We don’t know yet where she was coming from because we have not yet been able to get a statement from her,” the officer said. “It was determined it was a forced landing due to engine trouble, but we are not sure what engine trouble she was having or experiencing.”

Ontario Police received calls from witnesses around 9:46 a.m. about a small plane that had landed in the dairy property off Ontario Ranch Road and Broadway Avenue, just east of Archibald Avenue.

Officers spotted the plane rested against a metal fence with its left wing tipped onto the dirt.

“She was coming southbound over the houses,” Officer Payton said. “She was really low. We know that she went under the hanging electrical wires and missed the power poles. So, it was really good flying for her.”

The dairy owner told Ontario Police officers the cows had been moved off the property just two weeks ago.

The National Transportation Safety Board will work with the Ontario Police Department on the investigation, Officer Payton said.

On August 21, 2021, about 1330 eastern daylight time, an Enstrom 280C, N9285, was substantially damaged when it was involved in an accident in Daytona Beach, Florida. The pilot was not injured. The helicopter was operated as a Title 14 Code of Federal Regulations
Part 91 personal flight.

According to the pilot, he conducted a local flight around the airport and was approaching to land. Just prior to touchdown, the helicopter made an uncommanded climb and started to spin to the right. The pilot applied full left pedal, but the helicopter continued to spin to the right. He lowered the collective and the helicopter landed hard on the skids. The left skid was damaged during landing and the tail rotor contacted the ground damaging the tail rotor blades.

Examination of the helicopter revealed that the tail rotor drive shaft was spinning inside the coupling and not connected by a through bolt. The coupling and a section of the drive shaft was forwarded to the National Transportation Safety Board’s Material Laboratory for further examination.

On August 25, 2021, about 1822 eastern daylight time, an Acro Sport II experimental airplane, N169BF, was substantially damaged when it was involved in an accident near Fishers, Indiana. The pilot was not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

The pilot reported that the purpose of the local flight was to continue the engine break-in period after the recent replacement of two cylinders. Based on the pilot’s statement and fueling documentation, the airplane’s 23-gallon fuel tank was topped-off before the flight. The pilot stated that he operated the engine at maximum speed (2,800 rpm) with a full-rich mixture setting throughout the flight, and that the airplane had a total loss of engine power “a little over an hour” after takeoff. A structural tube in the forward fuselage was bent when the airplane struck a street sign during the forced landing.

A postaccident examination revealed 2-3 fluid ounces of fuel remained in the fuel system. There was no evidence of damage observed to the fuel tank; however, there was evidence of a fuel leak within the engine compartment (structural tubes, firewall, control cable clamps) and on the exterior lower fuselage skins aft of the firewall. The evidence of a fuel leak included areas of fuel pooling, droplets of fuel dripping from control cable clamps, a fuel sheen on the firewall, and fuel streaking on the exterior lower fuselage skins aft of the firewall. Further examination revealed the fuel inlet fitting to the fuel servo was loose. Pressurization of the fuel system did not reveal any additional leak sources.

The experimental engine was a modified Lycoming IO-360 with oversized cylinders and pistons. The engine did not have a data plate installed or serial number. The engine remained attached to the firewall. Engine control continuity was confirmed from the cockpit controls to the throttle arm and mixture control at the fuel servo. An engine examination did not reveal any mechanical anomalies that would have precluded normal engine operation during the flight. Additionally, during a post accident test, the engine operated normally without any hesitation, stumbling or interruption in power. There was no evidence of a fuel system leak following the 5 minute engine test run. The fuel consumption rate at maximum engine power on the ground (static power) was about 13.69 gallons per hour at 2,260 rpm. Based on the engine test run results, the estimated fuel consumption rate while inflight at maximum engine power (2,800 rpm) was about 16.96 gallons per hour. The airplane had fuel endurance of about 1 hour 21 minutes if the engine was operated at maximum power with a full-rich fuel mixture and 23 gallons of fuel.

Based on air traffic control radar track data, the flight was airborne about 1 hour 15 minutes 24 seconds. The pilot reported that he did not have a prolonged taxi or engine runup before takeoff.
