Registration: N52PY
Aircraft: Rockwell 690
Aircraft Damage: Substantial
Defining Event: Landing gear not configured
Injuries: 1 None
Flight Conducted Under: Part 91: General Aviation - Personal

The pilot in the retractable landing gear-equipped airplane reported that during approach, the airplane's airspeed was high. He decreased the airspeed and "pushed hard on the down lever" to extend the landing gear. However, the pilot had inadvertently pushed the lever to the side, which "bound up the (lever) handle." The pilot decided to land with the landing gear retracted, and the airplane slid to a stop on the runway.

The airplane sustained substantial damage to the lower fuselage.

The pilot reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation.

Analysis

The pilot of the tailwheel-equipped airplane reported that, during takeoff, the airplane encountered a wind gust and veered left. The airplane exited the runway onto adjacent grass, and the left main landing gear collapsed.

The airplane sustained substantial damage to the left wing.

The pilot reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation.

The airport's automated weather observation station reported that, about the time of the accident, the wind was from 320° at 8 knots, gusting to 17 knots and variable from 290° to 350°. The airplane was departing from runway 3.

Probable Cause and Findings

The National Transportation Safety Board determines the probable cause(s) of this accident to be:
The pilot's failure to maintain directional control during takeoff in gusting crosswind conditions, which resulted in a runway excursion and the left main landing gear collapsing.

Findings

Aircraft
Directional control - Not attained/maintained (Cause)

Personnel issues
Aircraft control - Pilot (Cause)

Environmental issues
Gusts - Effect on operation (Cause)
Crosswind - Effect on operation (Cause)

The student pilot reported that, after being endorsed for his first solo flight, he made three approaches and three landings. During the third approach, the nose of the airplane yawed to the right, and he applied left rudder to correct the right yaw but was unsuccessful. About 15ft AGL, the pilot aborted the landing and applied full engine power but, "The aircraft jerked and continued to descend with right yaw." The airplane landed on the runway with the nose of the airplane oriented to the right of the runway heading. The airplane exited the right side of the runway and stopped after encountering the rough terrain.

The airplane sustained substantial damage to the tail and empennage.

The METAR at the accident airport reported that, about the time of the accident, the wind was variable at 4 knots.

The student pilot reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation.

The pilot reported that during takeoff initial climb, once above the tree line, he felt as though the airplane encountered, "a strong quartering tailwind." The stall warning horn sounded, and the airplane began to descend to the left. He was able to put the airplane's nose down, and the airplane touched down in the grass area that outlined the left side of the runway surface and bounced. He cut the engine's power and the airplane touched down again in the grass area on the left side of the runway and "went onto the nose-wheel and the left main. The pilot applied right rudder and he was able to steer the airplane back onto the runway and stop.

The airplane sustained substantial damage to the left wing.

The pilot reported that about the time of the accident, the wind at the accident site was calm.

The pilot reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation.

The pilot reported that, during a familiarization flight with a flight instructor in a recently purchased, retractable landing gear-equipped airplane, while demonstrating a soft field takeoff, the tower controller cleared him for takeoff "with no delay." He quickly taxied the airplane to the runway centerline, applied power, and pulled back on the yoke. He added that, in retrospect, the back pressure used was too abrupt. The airplane entered ground effect about 10 knots slower than normal and was unable to maintain flight in ground effect and he was struggling to maintain runway heading. The airplane touched down "abruptly several times." The airplane drifted to the left and he thought he had a positive rate of climb, so he retracted the landing gear. Additionally, he was "too aggressive" with control inputs and "caused a stall." The left wing struck the taxiway and the airplane came to rest on the ramp. The pilot reported that the instructor did not take the flight controls.

The flight instructor did not provide a statement to the NTSB.

The airplane sustained substantial damage to the left wing.

The pilot reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation.

The airport's automated weather observation station reported that, about 5 minutes after the accident, the wind was from 090° at 13 knots. The pilot was departing on runway 04L.

Date & Time: 09/12/2019, 1520 PDT
Registration: N5229F
Aircraft: Cessna 172F
Aircraft Damage: Substantial
Defining Event: Nose over/nose down
Injuries: 2 None
Flight Conducted Under:Public Aircraft

The flight instructor reported that, during landing, he directed the student pilot to initiate a go-around. During the climb, the instructor saw dust devils at the end of the runway, so he instructed the student to also initiate a slight left turn. The instructor then noticed power lines past the dust devils, so he decided to take the controls from the student and continued a shallow, 200 ft-per-minute climbing left turn to the south. While in the turn, the instructor saw other high-tension power lines to the south, so he decided to initiate a precautionary soft field landing to a plowed field adjacent to the runway. During landing, the nose landing gear touched down and the airplane nosed over.

The airplane sustained substantial damage to the right-wing lift strut and vertical stabilizer.

The flight instructor reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation.

An automated weather observation station, about 20 minutes before the accident, about 15 nautical miles north from the accident site, reported the wind was 310° at 6 knots, temperature of 97ºF, altimeter setting of 29.93. The calculated density altitude was about 4,237 ft. According to the Federal Aviation Administration density altitude Koch Chart, the airplane would have likely experienced about a 37% decrease to the normal climb rate, and about a 55% increase to the normal takeoff distance. The pilot reported that the wind was 140º at 6 knots. The airplane departed from runway 23.
