Analysis

The pilot reported that the helicopter was about to take off with a flight nurse, paramedic, and a patient onboard when she noticed a flash and flame reflection on the front left windscreen immediately followed by screams and the cabin filling with smoke. The pilot immediately reduced the throttle, pulled the fuel shutoff valve, and applied full rotor brake. The paramedic on board’s flight suit was on fire. The flight nurse on board managed to open the helicopter’s right-side door, unbuckle the paramedic, and pull her from the helicopter. The flight nurse extinguished the flames, called for additional help, and began cutting away the burned flight jacket and flight suit from the paramedic. In the paramedic’s left chest flight suit pocket were keys and a burnt lithium battery. The nurse reported that when she went to remove the battery to prevent further injury to the paramedic, the battery was extremely hot and smoking, and burned her hand. She threw the battery to the ground and continued to render care to the paramedic. Later two burnt lithium batteries lying on the ground next to the helicopter’s left skid were recovered. The pilot reported no preaccident mechanical failures or malfunctions with the helicopter that would have precluded normal operation. The operator’s parent company issued a safety alert to all of its air and ground ambulance companies warning of the dangers of improperly carrying or storing loose lithium and lithium-ion batteries used in personal electronic devices.

Probable Cause and Findings

The National Transportation Safety Board determines the probable cause(s) of this accident to be:
Overheating of two lithium batteries in the paramedic’s flight suit pocket resulting in an explosion and fire that seriously injured the paramedic as the helicopter was about to take off.

Analysis

The helicopter pilot reported that, while hovering in front of a hangar he erroneously increased collective pitch, panicked, and lost control of the helicopter. The helicopter subsequently impacted the top of a hanger and rolled over. The helicopter sustained substantial damage to the main rotor and tail boom. The pilot reported that there were no preaccident mechanical malfunctions or failures with the airplane that would have precluded normal operation.

On February 11, 2022, about 2006 central standard time, a Mooney M20J, N201VZ, was substantially damaged when it was involved in an accident near Katy, Texas. The pilot and passenger sustained serious injuries. The airplane was operated as a Title 14 Code of Federal Regulations (CFR) Part 91 personal flight.

A review of archived Federal Aviation Administration automatic dependent surveillance broadcast (ADS-B) data revealed that the airplane departed Castroville Municipal Airport (CVB), Castroville, Texas, about 1827 and was enroute to West Houston Airport (IWS), Katy, Texas. After overflying IWS about 1,075 ft GPS altitude, the airplane turned left on to a downwind leg for landing on runway 15. The airplane turned final about 650 ft and descended until it collided with a tree on a golf course about 600 ft before the approach end of the runway. The airplane came to rest about 500 ft short of the runway and sustained substantial damage to the left wing and empennage.

HOUSTON, Texas – A small aircraft crashed through trees and onto a golf course in west Harris County, according to Harris County Sheriff’s Office.

Deputies said they responded to scene shortly before 7:30 p.m. Friday night where they found the aircraft upside down on a golf course.

According to a tweet sent out by the Harris County Sheriff’s Office, the aircraft was found at Pine Forest Country Club located in the 18000 block of Clay Road Friday evening.

Sheriff Ed Gonzalez said there were two people--a man and a woman--were found inside of the damaged aircraft.

Authorities said it took more than an hour to extricate both individuals from the plane. They were airlifted to Houston-area hospitals in stable condition, Texas DPS said.

KPRC 2 Investigates has learned that the tail number on that aircraft is N201VZ, according DPS. According to the FAA, the aircraft is a Mooney M20J 201.

Officials say according to the latest flight records, the plane took off outside of San Antonio around 6:13 p.m. Friday evening. It was scheduled to land at the West Houston airport at 7:25 p.m., but never made it.

Authorities believe the aircraft went through some trees before ending up on the ground.

Investigators from the FAA and NTSB were headed to the scene.
