Analysis

The pilot was landing the tailwheel-equipped airplane on a grass runway. After touchdown, the pilot applied light differential brake pressure to slow the airplane before exiting the runway; however, the airplane nosed over and came to rest inverted. The rudder, vertical stabilizer, and upper wings sustained substantial damage. The pilot reported the airplane had no preimpact mechanical malfunctions or failures that would have precluded normal operation.

Probable Cause and Findings

The National Transportation Safety Board determines the probable cause(s) of this accident to be:
The pilot's failure to maintain control of the airplane during the landing roll, which resulted in a nose over.

Findings

Personnel issues Aircraft control - Pilot
Aircraft Brake - Incorrect use/operation
Aircraft (general) - Not attained/maintained

Injuries:1 None
Flight Conducted Under: Part 91: General Aviation - Personal

On April 30, 2020, about 1430 central daylight time, an Ercoupe 415-C, N2275H, sustained substantial damage when it was involved in and accident near Hallettsville, Texas. The pilot was not injured. The airplane was being operated as a Title 14 Code of Federal Regulations (CFR) Part 91 personal flight.

The pilot reported that he departed from Uvalde, Texas (UVA), about 1330 and his intended destination was Eagle Lake, Texas (ELA). About an hour into the flight, about 7 miles from the Hallettsville Airport (34R), the airplane began to vibrate and lose engine power. The pilot tried to analyze the problem by reducing engine power and looked for a place to land. The pilot realized that he could not hold altitude and reach the nearby airport (34R). While maneuvering to avoid power lines, the pilot landed the airplane in an open field. During the forced landing the airplane sustained substantial damage to the forward part of the fuselage. The pilot exited the airplane uninjured and called for help.

Initial examination of the airplane wreckage conducted by the responding Federal Aviation Administration inspector showed that the left fuel tank was nearly empty. Fuel samples taken showed and unmeasured amount of contamination in the fuel. A follow-up examination of the engine and wreckage is planned when the airplane is moved out of the field and to a secure location.

Injuries: 1 None
Flight Conducted Under: Part 91: General aviation - Instructional

On April 22, 2020, about 1110 mountain daylight time, a Czech Sport Aircraft SportCruiser, N802PS, was substantially damaged when it was involved in an accident at the Logan-Cache Airport, Logan, Utah. The pilot was not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 instructional flight.

The student pilot stated that the purpose of the flight was for him to make his second solo cross-country. He departed from South Valley Regional Airport, Salt Lake City, Utah about 0900 and completed two practice touch-and-go takeoffs and landings in Ogden, Utah. Thereafter, the pilot continued to Logan where he completed a full stop and shut down the engine. After a short time he started the engine, and during the run-up procedure he performed a magneto check, at which time the engine momentarily experienced a total loss of power. The pilot shut down the airplane and tried again after speaking with his instructor on his cell phone. During the second engine start, he thought that the fuel pump sounded much louder than usual and he shut the engine down. On the third engine start, the fuel pump remained louder than normal, and he noticed that the fuel pressure was fluctuating within the normal range, but would temporarily drop to 1.8 psi when he turned on the avionics. The fuel pressure recovered to 3.5 psi and he completed a normal engine runup.

The pilot further stated that he taxied to runway 28 and proceeded to depart. As the airplane reached about 50 to 100 feet above ground level (agl), the engine surged and he noticed that the fuel pressure had decreased to 1.0 psi (flashed red). He retarded the throttle and attempted to land straight ahead because he thought there was enough runway remaining. The airplane touched down hard, bending the left landing gear. The airplane incurred substantial damage to the fuselage.

Location: Athens, GA
Accident Number: ERA20CA171
Date & Time: 05/03/2020, 1050 EST
Registration: N231DW
Aircraft: Mooney M20K
Aircraft Damage: Substantial
Defining Event: Landing gear collapse
Injuries: 1 None
Flight Conducted Under: Part 91: General Aviation - Personal

Analysis

According to the pilot, while on approach, he lowered the airplane's landing gear and confirmed that the landing gear was down and locked by observing the panel and floor indicators in the cockpit; however, during the subsequent landing roll, the left main landing gear collapsed. The airplane veered off the left side of the runway into a ditch. Examination of the airplane revealed that the left wingtip spar was buckled, and the torque tubing associated with the left main landing gear was bent and broken. Review of the logbook records did not reveal any recent maintenance on the landing gear system. Due to the damage of the landing gear assembly, a landing gear retraction test or rigging check could not be performed.

Probable Cause and Findings

The National Transportation Safety Board determines the probable cause(s) of this accident to be:
The collapse of the left main landing gear for reasons that could not be determined based on the available information.

Findings

Aircraft Gear extension and retract sys - Malfunction
Not determined (general) - Unknown/Not determined
