Beachgoers at Huntington State Beach enjoying the bright and sunny afternoon were surprised by the sudden descent and crash of a small aircraft that had been towing an advertising banner.

City spokeswoman Jennifer Carey said the emergency call reporting the accident came in at 1:30 p.m. Friday. The plane crashed into the water near Lifeguard Tower 13, where the 2022 California Surf Lifesaving Junior Lifeguard Championship was taking place. Carey confirmed the pilot was rescued with minor injuries.

No other injuries were reported.

Carey said the California State Parks Department would lead the investigation into the incident. The city’s marine lifeguards, police and fire departments also responded.

By 3 p.m., public safety officials had cordoned off the area of the crash and retrieved the wreckage from the water.

Riverside resident Maritza Robles said she and her family arrived for an afternoon at the beach just as the plane was beginning to come down. Her kids were already in the water as the plane descended.

"[The pilot] seemed to be OK. He was on top and we saw the lifeguards right away went out there and got him,” said Robles. “He was already on the top of the airplane [after] it fell. I think he knew that it was coming down, so I think he reacted. By the time it was in the water, he was already coming out.”

Robles’ daughter, Marissa Aguilar, 10, said the plane was towing a banner and appeared to glide down into the water.

Mariana Chanfreau said she saw the plane as it went down because she had been watching the waters for the end of one of the lifeguard races so that her children could go out into the ocean.

“It was just coming really low and, at first, it looked like it might glide. We knew it was going to go in at a point, but it looked like he was going to glide. You couldn’t hear anything, so it just kind of glided and then nosedived. In seconds, you had like 200 lifeguards out there,” said Chanfreau.

Chanfreau said the pilot seemed to be able to walk when she saw him being loaded into the ambulance.

“It was one of those big banners. It was big and it was white and looked like it was low. At some point, you thought maybe it was [a maneuver] just to show the sign, but it just came down,” said Chanfreau.

One woman said she has seen the same plane flying by regularly, usually advertising local restaurants.

The cause of the crash was unknown as of Friday evening.

SAN JOSE, California - An airplane pilot of a small aircraft suffered serious injuries during a crash in San Jose Friday evening, officials say.

Officials with the Federal Aviation Administration said in a preliminary statement that a Piper PA-32-301 Saratoga crashed while departing from the airport around 7:30 p.m. 

Police said the male pilot was taken to a local hospital. No other injuries were reported. FAA did not identify the pilot.

Officials are reporting downed power lines on the 2100 block of Evelyn Avenue and are alerting nearby residents. The public is asked to avoid the area while crews respond to secure the downed power lines. 

In an update, San Jose Fire Department said no evacuations were issued, but that area residents were asked to shelter-in-place due to the downed power lines. 

Pacific Gas and Electric crews are at the scene. At around 8:45 p.m. officials estimated it would take two hours to get the electrical hazard under control. As of 10 p.m., police said power was still out for many homes.

MONTCALM COUNTY, Michigan - One pilot in West Michigan was forced to make a runway out of crops on Saturday morning after they experienced an engine malfunction in-flight.

State police from Lakeview said the small-crop duster plane pulled off a hard landing in Montcalm County around 10:20 a.m. after the pilot told them the engine had failed.

Authorities said the pilot was uninjured and was able to safely land the plane in a field of beans and wheat.

The cause of the engine failure remains unclear at this time.

The crash comes just a week after a Cessna aircraft went down in Oceana County, killing both the pilot and the passenger.

A preliminary report from the July 15 crash found the deceased pilot, 56-year-old Raymond Gundy of Muskegon County, had not passed training to secure an instrument rating (IR).

According to authorities, weather conditions at the time made visibility poor.

A small plane went down in a field near Wardsville on Wednesday night and the pilot had no reported injuries.

According to authorities, shortly before 7 p.m., the Jefferson City 911 Center was contacted by air traffic control in Springfield that a small plane could be landing at the Jefferson City Airport that was reportedly in distress.

The pilot reported low oil pressure and smoke in the cockpit. Controllers reported the plane had dipped below radar contact and the plane would be coming in from the south.

Initially, authorities believed the plane went down in Osage County. The Osage County Sheriff's Department began looking for the plane as the pilot was in contact with air controllers.

Not long before 8 p.m., the plane was found in a field near Friendship Road in Cole County.

The Cole County Sheriff's Department said the Missouri Highway Patrol was called in to do a report on the incident.

Analysis

The private pilot reported that he was on final approach for landing when he observed another airplane landing on the same runway. To maintain separation between the two airplanes, the pilot performed s-turns and retracted the landing gear to improve the glide distance. When he extended the landing gear for the second time in preparation for landing, the landing gear position indicator light did not illuminate. The pilot pushed the gear reset button and subsequently pulled back on the control stick to abort the landing, which resulted in the airplane exceeding its critical angle of attack and stalling 10 to 20 ft above the runway. A witness observed the accident airplane on final approach with the landing gear retracted and at a low airspeed. The airplane touched down, bounced back into the air, settled onto  the runway, and slid about 100 ft before coming to rest.

Postaccident examination of the airplane revealed that the landing gear handle was in the UP position. A mechanic who recovered the airplane reported that he overheard the pilot say he was distracted on final approach.

Probable Cause and Findings

The National Transportation Safety Board determines the probable cause(s) of this accident to be:
The pilot's failure to maintain adequate airspeed during an aborted landing, which resulted in an exceedance of the airplane's critical angle of attack and an inadvertent aerodynamic stall. Contributing to the accident was the pilot's distraction while on final approach.

Factual Information

On December 4, 2016, about 1525 Pacific standard time, an experimental Velocity XL/RG airplane, N84KJ, was substantially damaged after it contacted the runway with the landing gear retracted at Petaluma Municipal Airport (O69), Petaluma, California. The private pilot sustained minor injuries. The airplane was registered to and operated by the pilot under the provisions of Title 14 Code of Federal
Regulations Part 91 as a personal flight. Visual meteorological conditions prevailed, and no flight plan was filed. The local flight originated from O69 at 1455.

The pilot reported that the purpose of the flight was to practice maneuvers required to obtain a commercial pilot certificate. Upon his return to O69, the pilot entered the downwind leg for runway 29, extended the landing gear, and prior to base leg, reduced the power to idle. He also observed a Cessna airplane on final. To keep an adequate separation between the two airplanes, the pilot announced that he would perform S turns. After he had executed two S turns, the pilot realized he was low and slow, so he retracted the landing gear to improve the glide distance. He initially intended to do a go-around, but decided to land after the Cessna exited the runway. As he attempted to extend his landing gear, he observed that the landing gear down position-indicator light did not illuminate. The pilot pushed the gear reset button and subsequently pulled back on the control stick, which resulted in an inadvertent stall 10 to 20 ft above the runway.

Postaccident examination of the airplane by a Federal Aviation Administration inspector revealed that the landing gear handle was in the UP position.

A witness observed the accident airplane on final approach, about one-third of the way down the runway with the landing gear retracted and at a low airspeed. He immediately started flashing lights in his vehicle to get the pilot's attention. The airplane touched down, bounced back into the air, settled on its belly and slid about 100 ft to a stop.

A mechanic who recovered the airplane reported that the landing gear handle was in the UP position, and that he overheard the pilot say that he was distracted on final approach. Maintenance personnel on scene noticed the gear was partially out of the well before they started to lift the aircraft during the recovery process.

In the section titled "RECOMMENDATION" in the NTSB Pilot/Operator Report, form 6120.1, the pilot stated that the decision to proceed with the landing proved to be inadequate and that he should have performed a go around instead.

The pilot completed necessary repairs on the airplane that included fiberglass repair to the belly and the landing gear system. The nose gear actuator and strut were replaced. The landing gear was then functionally tested and there were no malfunctions.
