Analysis

The pilot reported that, he had just landed when he thought that he saw a crop duster airplane descending for a short base for landing on the opposite runway. The pilot initiated a go around with the flaps still extended and with a high-density altitude. The airplane attained an altitude of about 50 to 100 ft above ground level when the airplane stalled, and the left wing dropped. The pilot attempted to recover but did not have enough altitude before the airplane collided with the ground. The airplane nosed over and came to rest inverted. The wings and fuselage were substantially damaged. The pilot reported that there were no preaccident mechanical
failures or malfunctions with the airplane that would have precluded normal operation.

Probable Cause and Findings

The National Transportation Safety Board determines the probable cause(s) of this accident to be:
The pilot's exceedance of the airplane's critical angle of attack during a go-around, which resulted in an aerodynamic stall and subsequent impact with terrain.

Findings

Personnel issues Decision making/judgment - Pilot
Aircraft Angle of attack - Not attained/maintained
Personnel issues Aircraft control - Pilot

Incident occurred December 24, 2021 in Lyme, Jefferson County, New York

LYME — A plane was forced to make an emergency landing after a mechanical issue on Friday, which was witnessed by a woman who saw the pilot signaling from the aircraft.

Dorry McIntosh was inside her home Friday afternoon when she witnessed the plane go down. She said it had been flying in her area for much of the day. She said she saw it fly in front of her house and then over a field across the road.

The plane then did a somersault motion, she said, before something might have gone wrong. She said she saw the pilot waving his hands from the plane as if he was signaling to them that the aircraft was going down. And then it did, landing in the field across the street from her. She said her husband immediately dialed 911.

At around 4:32 p.m., Jefferson County sheriff’s deputies responded to the report of the small plane that had crashed in a field near 25717 Moffatt Road in the town of Lyme. An investigation revealed that a 1998 Lockwood Drifter with a solo pilot onboard was forced to make an emergency landing due to mechanical issues.

The landing was successful and the plane sustained minor damage. There were no injuries reported. The Federal Aviation Administration was notified and will follow up as necessary. The sheriff’s office was assisted by state police, the Chaumont Volunteer Fire Department and the Cape Vincent Ambulance Squad.

Location: Twin Falls, Idaho
Accident Number: WPR21LA264
Date and Time: June 30, 2021, 20:45 Local
Registration: N9609H
Aircraft: CHRISTEN INDUSTRIES INC A-1
Aircraft Damage: Substantial
Defining Event: Loss of engine power (partial)
Injuries: 1 None
Flight Conducted Under: Part 91: General aviation - Personal

Analysis

The pilot reported that he was practicing basic flight maneuvers at an altitude of about 3,500 ft agl during a short uneventful local flight. He then reduced engine power to idle and initiated a descent back to the private dirt airstrip with the intention of first performing a low pass to confirm the runway was clear, and then landing. As he flew past the runway, he applied full throttle, but the engine did not respond. He then performed a forced landing into an adjacent wheat field, where the airplane nosed over during the ground roll. The pilot stated that he had not used a checklist during the descent and forgot to apply carburetor heat. The wing lift struts, and the vertical stabilizer were substantially damaged. The pilot reported that there were no mechanical failures or malfunctions with the airplane or engine that would have precluded normal operation.

The carburetor icing chart from FAA Special Airworthiness Information Bulletin CE-09-35 “Carburetor Icing Prevention” indicated a potential for serious icing at glide power for the temperature and dew point depicted at the time of the accident.

Probable Cause and Findings

The National Transportation Safety Board determines the probable cause(s) of this accident to be:
Partial loss of engine power during a low pass, due to carburetor icing and the pilot’s failure to use carburetor heat during the landing approach.

Findings

Personnel issues Use of checklist - Pilot
Personnel issues Forgotten action/omission - Pilot
Aircraft Intake anti-ice, deice - Not used/operated
Environmental issues Conducive to carburetor icing - Effect on equipment

Ground Handling Event: Aeronca 11AC, N9149E; accident occurred July 05, 2021 at Neil Armstrong Airport (KAXV), Wapakoneta, Auglaize County, Ohio

In a follow-up to this accident, the damaged Aeronca 11AC, N9149E, has been sold and Ted Bergstrom, airport manager, said the anticipated removal date is October 14, 2021. The original aircraft owner had elected to take the insurance settlement rather than repair the damaged wingtip. Since the auction was done as a blind bid, he had no figures on what the successful bid was. The airport manager said the airplane owner’s insurance was billed for $545 for hangar rental and $900 for repair of the hangar door.

Analysis

The airplane was not equipped with an electrical system and the engine was started by hand propping. The pilot reported that he did not have a passenger to hold the brakes in the cockpit during engine start, nor did he use wheel chocks and/or tether the tail to a fixed object because he thought using the airplane’s parking brake would keep the airplane stationary after the engine had started. Before the engine was hand propped, the pilot set the throttle about ¼” to ½” forward of the idle position, but the engine’s speed was much higher than he had expected after the engine started. The unoccupied airplane initially moved forward before it entered a left turn on the ramp in front of a row of hangars. Although the pilot was able to reach into the cockpit to move the throttle to idle, the airplane continued in the left turn and struck a closed hangar door, resulting in substantial damage to the right wing. The pilot did not report any mechanical malfunctions with the airplane that would have precluded normal operation.

Probable Cause and Findings

The National Transportation Safety Board determines the probable cause(s) of this accident to be:
The pilot’s inadequate engine hand propping procedure resulting in the unoccupied airplane’s unintended movement and subsequent impact with a hangar.

Findings

Aircraft Tie-down/mooring - Not installed/available
Aircraft Landing gear brakes system - Capability exceeded
Personnel issues (general) - Pilot
Personnel issues Use of equip/system - Pilot

Analysis

The pilot reported that the airplane was at an excessive speed on final approach and landed hard. After the hard landing, the airplane bounced and touched down a second time. The pilot was unable to regain control of the airplane and it veered left off the runway and came to rest in a grassy area nose down. The pilot added that there were no preimpact mechanical malfunctions. Examination of the airplane by a Federal Aviation Administration inspector revealed substantial damage to the empennage and right wing.

Probable Cause and Findings

The National Transportation Safety Board determines the probable cause(s) of this accident to be:
The pilot’s failure to maintain directional control while landing, which resulted in a runway excursion and collision with terrain.

Findings

Personnel issues Aircraft control - Pilot
Aircraft Directional control - Not attained/maintained
