On June 25, 2022, about 1823 central daylight time, a Mooney M20J, N4267H, was destroyed when it was involved in an accident near Kerrville, Texas. The pilot and passenger were fatally injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 flight. 

The airplane had undergone an annual inspection that was completed in December 2021. The accident pilot was unable to pick up the airplane following the inspection, so it remained with the maintenance facility until the day of the accident. The accident flight was the first time the  pilot had flown the airplane since the inspection.

Because the airplane sat unused for so long, in advance of the arrival of the pilot, the maintenance facility flew a test flight on June 23. During that flight, it was discovered that the directional gyro was inoperative. The gyro was replaced the following day, and another test flight was flown. The test pilot stated that beyond the problems with the directional gyro, the test flights were uneventful.

The airplane was then serviced with the addition of 25.1 gallons of 100 low-lead aviation fuel to the fuel level indicators, or “tabs”, which would have indicated that each tank contained 25 gallons of fuel (32 gallons capacity).

The pilot arrived at the airport about 1700 on the day of the accident, having been flown inbound along with the passenger from Dublin Municipal Airport (9F0) by a friend.

None of the security cameras located at Kerrville airport recorded the airplanes takeoff, and no witnesses came forward to report they had observed the initial takeoff sequence. However, the airport manager stated that runway 12 was active at the time of the accident.
Preliminary ADS-B data provided by the FAA indicated that at 1819, the airplane was on an east-northeast track, about 2 miles east of the departure end of runway 12 at an altitude of about 1,775 ft mean seal level (15o ft above ground level (agl)). When combined with field wind conditions, the data revealed that the airplane was travelling at an airspeed of about 70 knots. 

Over the next two minutes, the airplane began a 180° left turn, while travelling at about 80 knots. The airplane began to accelerate as it rolled out of the turn, reaching 96 knots as it approached runway 30. The last recorded target indicated the airplane was about 150 ft agl, 1,700 ft short of the runway 30 threshold, while tracking on the runway heading.

About this time, a witness, who was also a pilot, was driving eastbound on a parallel highway, just south of the runway. He stated that he was passing about midfield, when he saw a Mooney airplane approaching runway 30 with the landing gear extended at an altitude of about 150 ft agl. He knew the winds that day were favoring the southeast and wondered why the airplane was making what appeared to be a downwind landing on runway 30. He thought the pilot may have been practicing an instrument approach but noticed that the airplane appeared to be flying erratically in both the lateral and vertical planes. He stated that the propeller was turning, and there was no fire, smoke, or other signs of distress. As the flight progressed, the airplane became offset to the north of the runway centerline while maintaining altitude, he then lost sight of the airplane as he continued to travel along the highway.

A southeast-facing security camera located on the wall of a school, about 0.75 miles northwest of the departure end of runway 30, captured the last 2 seconds of the flight. The video showed that the airplane was flying southwest just above a tree line while in a left 30° bank. The airplane continued to roll left until the underside was visible as it descended into the trees and a fireball ensued.

The airplane came to rest at an elevation of about 1,600 ft on the slope of a hill, about 4,200 ft beyond the runway 12 threshold, and 20° offset the runway centerline (figure 1). The first identified point of impact was a cleanly cut swath of tree branches and limbs, at an angle of 70° relative to the horizon. The airplane was located about 30 ft southwest of the swath, at the base of a felled 50-ft tall oak tree. The tree exhibited impact damage along its north face.

The cabin skins were consumed by fire, with only crushed and bent steel frame members remaining. Most of the wings and the tail section were thermally consumed, however, sections of all major airframe components and primary flight controls were found within the immediate wreckage site. The landing gear actuator was located, and its jack screw appeared to be fully retracted, consistent with the landing gear being extended at impact. The trim jack screws in both the moving tail, and elevator cabin trim assemblies exhibited extension dimensions that matched the takeoff pitch trim position.

The woman killed along with the pilot in Saturday’s plane crash near the Kerrville-Kerr County Airport was 47-year-old Ruthy Finch, of Dublin,  Erath County,  Texas.





Two fatalities have been confirmed following a fiery plane crash that occurred Saturday evening near the Kerrville-Kerr County Airport.

According to reports by the Federal Aviation Administration, the pilot and a passenger were killed when the aircraft crashed under unknown circumstances and caught fire.

“A Mooney M20J 201 crashed after departing from Kerrville Municipal Airport/Louis Schreiner Field in Kerrville, Texas, around 6:30 p.m. local time Saturday. Two people were on board. The FAA and the National Transportation Safety Board will investigate. The NTSB will be in charge of the investigation and will provide additional updates,” an FAA spokesperson said.

According to Kerrville Fire Department Chief Eric Maloney, the call was received by his department at 6:27 p.m. reporting a plane crash west of the airport off Peterson Farm Road.

At the time of the call, one fatality was confirmed and a large, fast-moving brush fire was fully engaged.

“The accident was 50 yards east of OLH, (Our Lady of the Hills College Prep) with the brush fire moving toward the school,” Maloney said. “The brush fire was ignited from the crash and the aircraft fuel.”

Maloney said KFD Engine 1 was first to arrive and was able to connect to a fire hydrant near the head of the fire to establish a quick attack with “handlines and the master stream to knock down the fire from making forward progress toward the school.”

A second engine was also dispatched to make access to the aircraft and extinguish the fire, Maloney said.

“At that time, we did identify there was a second fatality in the aircraft,” Maloney said.

Maloney said mutual aid was requested for brush trucks from the Center Point and Turtle Creek volunteer fire departments.

“Approximately, five acres burned, but no structures were damaged from the brush fire,” Maloney said. “The quick actions of KFD in coordination with our VFDs allowed us to contain the brush fire and stop it from spreading to the school or surrounding acreage of high grass and brush.”

Maloney said KFD worked in coordination with FAA and the Department of Public Safety to assist in extricating the victims from the wreckage.

According to a spokesperson for the National Transportation Safety Board, a preliminary investigation into the cause of the crash will be complete within 12 days. The final report will take approximately 12-18 months, he said.

The identities of the victims have not yet been released, however, the plane is registered to GMT Enterprises, LLC, of Stephenville, Texas, owned by Greg Thompson.

A social media post by the Stephenville Texas Area Pilots and Aviation Enthusiasts identified Thompson as one of the victims.

On June 24, 2022, about 0903 mountain daylight time, a Cessna 152, N934JH, sustained substantial damage when it was involved in an accident in Mendon, Utah. The flight instructor and pilot receiving instruction sustained fatal injuries. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 instructional flight.

According to the flight school’s chief pilot, the purpose of the flight was to conduct spin training for the pilot receiving instruction. The accident airplane departed Logan-Cache Airport, Logan, Utah. Preliminary ADS-B data provided by the FAA indicated that the airplane traveled to an aviation practice area about ten nautical miles southwest of the airport. The airplane maneuvered in the area for about 30 minutes prior to the accident. Surveillance video footage of the accident sequence was located and showed the airplane in a fully developed spin prior to impacting terrain.

The airplane wreckage was located on upward sloping terrain in an agricultural area. The airplane in its entirety came to rest on a heading of 223°. The airplane impacted terrain in a nose down attitude, and the propeller hub with both propeller blades attached, separated from the propeller flange. The left and right wings were compressed forward toward the engine, emanating at the wing roots. The flaps and ailerons remained attached to their respective wing, in their entirety. The horizontal and vertical stabilizers remained attached in their entirety. The nose landing gear collapsed and folded aft, underneath the cabin seats. The left and right main landing gear remained attached to the lower fuselage, aft of the cabin.

Michael Allan Carpenter

Our beloved son, brother, uncle and friend Michael Allan Carpenter, 25, passed away doing what he loved on June 24, 2022 near Logan, Utah.

Michael was born in Sanford, FL to Tom and Sandy Carpenter but spent most of his childhood years in Spanish Fork, UT. Growing up, Michael had a zest for life that couldn’t be matched. He was forever off on one adventure or another. He thrived on being in the outdoors in any capacity he could, whether it was snowboarding or hiking and camping, he loved it all.

As a young adult, Michael joined the Air Force and was a member of the 151st Air Refueling Wing in the Utah Air National Guard and deployed in support of Operation Inherent Resolve. While attending Utah State University in Logan, Michael discovered a passion for flying. His plan was to become a commercial pilot and he was in the process of completing his last certification when he passed away.

He will be remembered for his generous and kind heart, his quick wit, and his love for life. Michael was loved and respected by all who knew him. Michael is survived by his parents, Tom and Sandy, his sisters, Kyrie (Brad) and Kori (Stefan), his brother, Kaleb, as well as three nieces, a nephew, and countless cousins.

Services for Michael will be held on Saturday, July 2, 2022 at the Canyon Ridge Stake Center, 1911 E 1850 S in Spanish Fork, Utah. Visitation will start at 9:30 am, followed by a funeral service at 11:00 am. Interment will be at the Spanish Fork Cemetery immediately after the service.


Blake Lamar Shumway
July 28, 1997 — June 24, 2022

Blake Lamar Shumway gained his heavenly wings on June 24, 2022, doing something he loved immensely, flying. Blake was born to Adam and Lisa Shumway on July 28, 1997. He was raised in Syracuse, surrounded by family and friends. Blake was known for his bright orange curly hair and did not have any problem letting it be known it was orange and not red! He was also known for his goofy personality and for being the life of the party. 

Blake attended Cook Elementary, Syracuse Jr. High, and Syracuse High School and graduate in 2015. While attending high school, Blake participated on the soccer and lacrosse teams. Soccer was a big part of Blake’s life. At the age of 8, he started competing on competition soccer teams, creating many lifelong friendships. He loved people and was a friend to everyone. 

Blake loved life and was extremely driven to reach his goals. He was often not found at home. Any free time he had he was hiking, rock climbing, skiing, flying, or finding something to do in the great outdoors. He also loved camping with his family, backpacking, and fishing. He had a special bond with his brother and sisters and loved going on adventures with them. Blake also loved sitting on the cousins’ couch, playing board games, and playing sports in Grandma’s yard with all his cousins.

He was an active member of The Church of Jesus Christ of Latter-day Saints. He served a mission in Kennewick, Washington. His flight home was the first time he flew on an airplane. That was when he discovered his life’s mission: to become a pilot. Blake quickly grew a passion for flying. He received his CFI in April of 2020 and began teaching as a flight instructor at Utah State University in September of 2021. He loved learning everything he could about flying and shared it with anyone who would listen. He often talked about becoming an aerial wildland firefighter pilot.

From the first time Blake met Susannah Woodbury, he knew there was something special about her. They were made for each other. Her sense of adventure matched his perfectly. They were sealed for time and all eternity on June 18, 2021, in the Logan Utah temple. They lived life to the fullest and never let a minute go to waste.

Blake was preceded in death by his grandparents, William and Wanda Shumway; two aunts; one uncle; and two cousins. 

He is survived by his wife, Susannah Woodbury; in-laws, Don and Dearwyn Woodbury; parents, Adam and Lisa Shumway; brother, Garrett; sisters, Shannon, Camrey, and Racail; his grandparents, Lamar and Rosella Paice, as well as many aunts, uncles, cousins, and friends. 

A visitation will be held on Wednesday, June 29, 2022, from 5:00-8:00 p.m. at Lindquist’s Syracuse Mortuary, 869 S. 200 W., Syracuse, Utah. Funeral Services will be held Thursday, June 30, 2022, at 11:00 a.m. at Solitude Mountain Resort’s Last Chance Lodge, 12000 S. Big Cottonwood Canyon Rd, Solitude, UT. Please proceed to the 2nd Solitude entrance (Village Lot) and enjoy a short walk to The Last Chance Lodge. Overflow parking will be in the Moonbeam Lot (1st entrance). The funeral will be held outdoors unless there is inclement weather, in which case we’ll go inside the lodge. Please plan for sun and potential weather and bring an umbrella.   

Interment will follow at the West Point Cemetery. 






MENDON, Utah  — Law enforcement has confirmed two members of the Utah State University Aviation Program died Friday morning, when the small plane they were flying in went down in a field between Mendon and Wellsville. The crash was reported around 9:00 a.m.

Cache County Sheriff’s Lt. Mikelshan Bartschi said the instructor plane went down in a field west of 2831 S. State Road 23, near the base of the Wellsville Mountains. Witnesses quickly arrived at the crash site and assisted in guiding law enforcement and EMS personnel onto the scene. It was learned by the witnesses that there were no survivors of the crash.

Utah State Spokesperson Amanda DeRito confirmed the identity of the victims as USU flight instructor Blake Shumway and aviation student Michael Carpenter. USU Police have notified both men’s families.

Shumway was a certified flight instructor and had worked for USU Aviation since September 2021. Carpenter was majoring in aviation technology, to become a professional pilot and was scheduled to graduate in the fall.

“We are devastated by this morning’s tragic crash and the loss of members in our USU community,” said USU President Noelle E. Cockett. “Every student and employee is part of our Aggie family, and we know many individuals will feel this loss deeply.”

DeRito said other USU Aviation flights were immediately grounded. The school will be working with employees and students in the program to offer counseling and other services to help them process this loss.

Bartschi explained that the sheriff’s office originally cordoned off the area, interviewed witnesses and collected evidence at the crash site. They were assisted by investigators from Utah State University, victims’ services from The Cache County Attorney’s Office and from USU. The Utah Highway Patrol also helped with scene documentation.

The Federal Aviation Administration (FAA) and National Transportation Safety Board (NTSB) will investigate the cause of the crash.

“The Sheriff’s Office offers its heartfelt condolences to the families and loved ones of the occupants,” expressed Bartschi. “Please keep the families of this crash in your thoughts and prayers.”

Analysis

The pilot reported that he initiated a takeoff roll on the 2,200-ft-long wet, turf runway, and that, about 1,650 ft down the runway, the airplane encountered standing water, and the speed decreased. He reported that he "still felt I had plenty of room to regain airspeed and take off safely." The airplane encountered standing water again on another section of the runway, and the speed decreased further. The pilot retracted two notches of flaps and attempted to climb the airplane, but it exited the departure end of the runway and entered a cornfield.

The airplane sustained substantial damage to the engine mounts.

The pilot reported that there were no mechanical malfunctions or failures with the airplane that would have precluded normal operation.

On June 27, 2022, about 1002 mountain daylight time, an experimental amateur built Van’s RV-9A airplane, N12VV, was substantially damaged when it was involved in an accident near Missoula, Montana. The pilot was seriously injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

According to the accident pilot (and builder of the accident airplane), he was performing flight tests on his airplane to break-in the engine. While approaching Missoula Montana Airport to land he experienced a total loss of engine power.

Preliminary air traffic control audio revealed that the pilot contacted air traffic control about 5 minutes prior to the accident to request permission to enter the airspace and perform a full stop landing. The controller instructed the pilot to enter the left downwind leg of the airport traffic pattern for runway 30. Approximately 4 minutes later the accident pilot informed the controller that his engine was “surging” and “probably about to quit” and the controller cleared the pilot to land on runway 08. An airport surveillance video showed the airplane descending from a low altitude as it approached the airport from the northwest. While passing near the approach end of runway 08, the airplane made a steep left turn from an estimated altitude of about 30 ft above ground level and descended rapidly towards the ground and impacted terrain. The airport manager stated that the wreckage came to rest between runway 12/30 and taxiway alpha between midfield and the approach end of runway 30. The wreckage was located about 0.5 nm east of the approach end of runway 08.
