On February 26, 2021, about 1444 eastern standard time, a Cessna 172N, N5631E, was substantially damaged when it was involved in an accident near Boynton Beach, Florida. The flight instructor and student pilot were not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 instructional flight.

The flight instructor and the student pilot provided written statements that were consistent throughout. Approximately 10 minutes after departure, the student pilot was performing ground reference maneuvers at 1,000 ft when the flight instructor noticed the airplane had descended “about 100 ft.” He advised the student pilot to climb the airplane back to the desired altitude, but the airplane did not respond to the student’s control inputs.

The flight instructor assumed control of the airplane and performed remedial actions to restore engine power, which were unsuccessful. He then selected a dirt road that bisected cultivated fields for a forced landing. After landing, the airplane’s right wing, right main landing gear, and right-wing strut struck a gate post along the road, which separated the gear and strut, collapsed the wing at its root, and rotated the airplane 180° before it came to rest on its right side.'

Initial review of maintenance information revealed that the airplane’s most recent 100-hour inspection was completed on February 13, 2021 at 15,942 total aircraft hours.

The airplane was recovered for further examination.

One man was injured after a plane crash at the Wakulla County Airport in Panacea Sunday evening.

The Wakulla County Sheriff's Office and EMS responded to the crash, posting about it on Facebook around 6 p.m. Sunday. The department initially described the crash of the small unidentified aircraft as one "with injuries" but provided no specifics.

In an update released Monday afternoon, the department said: "Emergency personnel were able to remove the pilot who was the sole occupant of the plane from the wreckage. The pilot was then transported to a local hospital with non-life-threatening injuries."

WCSO said the FAA responded and would be conducting the investigation moving forward.

As word spread about the crash on social media Sunday, Facebook commenters said they had heard the plane's pilot would be alright. 

A photo posted on Facebook showed the scene of the crash. The fuselage of the small plane appeared intact, but Tallahassee resident Rick Oppenheim, who was passing by, said the landing gear were "flattened."

"Looked like 1 person was lying on the ground," he wrote on Facebook. "We counted 3 Highway Patrol cars, 6 Wakulla Sheriff’s cars, 3 fire trucks and 1 EMT passing us heading there."

On February 26, 2021, about 1540 central standard time, a Robinson R44 helicopter, N7184N, was substantially damaged when it was involved in an accident near Dunn, Texas. The pilot and two passengers were not injured. The helicopter was operated as a Title 14 Code of Federal Regulations Part 91 aerial gunning flight.

The pilot reported that he had just refueled the helicopter and departed again with the fuel tanks about half full. While flying at 80 ft above ground level (agl), he heard the “rotors droop” and “engine bog down.” He applied right pedal and increased the throttle input, but the engine did not respond. The helicopter descended into the trees.

The helicopter impacted the ground and sustained substantial to the fuselage. Figure 1 shows where the helicopter came to rest upright among trees. 


The helicopter has been retained for further examination.

On February 27, 2021, about 1031 central standard time, a Piper PA-23 airplane, N1460P, was substantially damaged when it was involved in an accident in Oshkosh, Wisconsin. The pilot and flight instructor were not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 instructional flight.
  
The flight instructor reported the routine training flight included airwork which involved numerous landing gear extension/retraction cycles. During the final 45 minutes of the flight, the nose landing gear began to indicate down and locked, and then unsafe. Several touch-and-go landings were performed during which he attempted to “lightly move” the nose wheel into the down and locked position. 
  
Contact with the runway pavement was firm enough to indicate that the nose landing gear was down and locked. After touching down, the airplane began yawing to the right with an “unusual noise.” The application of left brake prevented the airplane from leaving the runway pavement; however, the nose landing gear collapsed shortly afterward. The airplane traveled an additional 100 to 200 ft. resting on the nose before coming to a stop on the runway.
  
Airframe damage included multiple fuselage bulkheads, skin panels and the nose wheel well structure.
  
An examination of the landing gear is planned.
