The flight instructor reported that, he was providing simulated instrument flight instruction to a private pilot rated student. Following the instruction, due to increased wind and rain and decreasing visibility the instructor took the controls while returning to the airport.

Prior to touchdown, the instructor kept the airplane in ground effect to reduce the airspeed and the airplane touched down about halfway down the 3,578 ft. runway, bounced twice, and touched down again. He then applied brakes and the student "got the flaps up." After exiting the runway onto a taxiway, the instructor applied brakes but the "airplane was still traveling too fast when [it] entered the ramp" and veered slightly left of taxiway centerline striking three unoccupied airplanes on the ramp.

The airplane sustained substantial damage to the left-wing and left-wing spar box.

The instructor reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation.

The automated weather observation station located on the airport reported that, about the time of the accident, the wind was from 200° at 16 knots, gusting 16 knots, 1 statute miles, broken clouds at 3,900 ft, temperature 81°F, dew point 73°F, with heavy rain. The airplane landed on runway 27C.

Family, friends, planes, motorcycles, snowmobiles, heavy equipment and epic parties at the river ... these were a few of Ken's great loves in life. It is with heavy hearts and sadness that we say goodbye. On May 6th, 2019, Kenneth W. VanAssche, 69, of Chelan, Washington, died shortly after take-off at the Chelan Airport in his Just JA30 SuperSTOL experimental aircraft. It was a beautiful day and he was ready to soar. After learning to fly recently, he wanted to check flying off his "bucket list of life". His list was long, exciting and filled with adventures. The stories of his life were great and we have lost a true legend among us.

The pilot reported that as he made the approach over a river to land at his private airstrip, he "came in too low and hit a dock," that was moored to the side of the river. The airplane's propeller struck the dock and the landing gear separated from the airplane which slid about 25ft on the airstrip and came to a stop.

Analysis

The pilot of the tailwheel equipped airplane reported that, after a wheel landing, the tail wheel touched down on the ground and started a "violent shaking." He pushed forward on the control yoke to raise the tail, increase throttle and subsequently let the tail touched down on the runway again. The tailwheel continued to shake, and the airplane began to veer left but the pilot corrected with opposite rudder. Shortly after the airplane regained runway heading, the airplane veered right, exited the runway to the right and ground looped.

The airplane sustained substantial damage to the left wing.

The pilot reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation.

The pilot added that, during landing, the wind was from 010º at 4 knots. The automated weather observation station located on the airport reported that, about the 15 minutes before the accident, the wind was from 160° at 4 knots. The airplane landed on runway 03.

According to the pilot, she had flown from her home airfield to another airport to receive instrument training. The departure to, and arrival at, the other airport had been uneventful, however during the training she and the instructor noted that the left brake was less effective than the right brake. The brakes had been serviced two days prior to the flight. Following the training, she returned to her home airport, and during landing on the wet grass runway, she aborted the landing because there was insufficient runway to safely stop the airplane.

During the second landing she touched down with about 2/3 of the 2,600-foot-long runway remaining. During the landing roll she ensured that the throttle was in the idle position, and she retracted the flaps, and applied aft pressure to the yoke.

She applied the foot brakes and then the hand brake, and again noted that the left brake was less effective than the right, but the airplane continued to slide on the wet grass. The airplane overran the runway and encountered a drainage culvert. Subsequently, the right wing struck a barn, and the left wing struck a trailer.

The airplane sustained substantial damage to both wing's spars and ribs.

According to the Federal Aviation Administration, Aviation Safety Inspector that examined the airplane, there had been a pool of hydraulic fluid on the ground that appeared to be consistent with an O-ring failure or displacement. He affirmed that although degraded, the brake would have been functional yet requiring a pumping action by the pilot to build pressure within the brake line.

Two days prior to the accident, the pilot's grandfather noticed that the airplane had a "soft left brake." He flew the airplane to a maintenance facility and the brakes were serviced by adding brake fluid to the brake system and the brakes were bled. The grandfather flew the airplane home and was satisfied with the brake operation.

According to the pilot he had sought instruction from a flight instructor to complete his airplane single engine land, commercial certificate. He completed his airplane multi-engine land, commercial certificate two years prior, and his airplane single-engine land, private certificate three years prior.

He reported that during his solo flight, he made an approach, but the airplane's airspeed was fast. He recalled that his intent was to float along the 8,000ft runway, extending the distance to touchdown because he wanted to exit at the taxiway near the end of the runway.

He reported that during the flare, he misperceived the airplane's height above the runway, because the runway was 50ft wider than his departure airport's runway. The airplane "sunk", touched down on the main landing gear, and bounced.

The pilot reported that he panicked and applied back pressure to the yoke, "thinking that it would slow me down." The airplane bounced several times and he applied forward pressure to the yoke, "to get some forward momentum to flare again." The airplane settled on the runway.

During taxi the pilot felt something was wrong with the nose wheel but continued to taxi to the hangar.

The airplane sustained substantial damage the lower engine mounts.

The private pilot reported that he was receiving commercial instruction in an airplane with retractable landing gear during a cross country flight to an unfamiliar airport. He added that while he turned the airplane from the downwind leg to the base leg of the traffic pattern for the runway, he did not perform the checklist, and the flight instructor did not verity that the landing gear were extended. The airplane touched down on the runway with the landing gear retracted.

The airplane sustained substantial damage to the external longerons on the lower fuselage.

The pilot reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation.

The pilot did not submit the National Transportation Safety Board Pilot/Operator Aircraft Accident/Incident Report Form 6120.1.
