On September 12, 2021, about 0900 Alaska daylight time, a Stinson 108-3 airplane, N6476M, was substantially damaged when it was involved in an accident near Port Alsworth, Alaska. The pilot was not injured. The airplane was operated by the pilot as a Title 14 Code of Federal Regulations Part 91 personal flight.

The pilot reported that he was landing at his private airstrip, and after touchdown, during the landing roll, the right brake “went flat” as he applied the brakes to slow the airplane’s ground roll. The airplane subsequently veered to the left, impacted a stump, and then it nosed over, sustaining substantial damage to the fuselage and left wing.

The airplane was equipped with Cleveland 30-52 double puck brakes.

An initial postaccident examination by the pilot revealed the right brake torque plate had fractured. A detailed examination of the torque plate is pending.

Reports of an aircraft emergency landing at Pitchess Detention Center in Castaic prompted a full response from law enforcement and first responders Tuesday, with initial reports indicating the plane’s occupants were not injured.

The incident was first reported shortly after 3 p.m. near Hasley Canyon Road and Interstate 5, according to Los Angeles County Fire Department representative Charisma Murrillo.

Initial reports from a passerby indicated a small plane had landed in a field, though that had yet to be confirmed by officials who had yet to arrive on the scene as of 3:15 p.m., Murrillo said.

Upon arrival, firefighters located the plane, which reportedly had a “hard landing,” Murrillo later added.

“We have a small-engine plane that did a hard landing on Pitchess Detention Center property,” said Deputy Eva Jimenez of the Sheriff’s Information Bureau. “The two male, adult occupants were uninjured and there was no damage to the plane.”

Jimenez said that initial reports indicated that the cause for the hard landing was engine troubles.

A number of first responder and law enforcement vehicles could be seen driving into the Pitchess Detention Center facility with lights on following the reported hard landing.

On September 14, 2021, about 1200 eastern daylight time, an experimental, amateur-built Zenith 750 Cruzer, N161AR, was substantially damaged when it was involved in an accident at Massey Ranch Airpark (X50), New Smyrna Beach, Florida. The pilot and pilot-rated passenger were not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91personal flight.

According to the pilot, he purchased the airplane on the day of the accident and intended to fly it back to Greenville, Tennessee. He conducted a short post-purchase inspection on the airplane and noted that there were no brakes or throttle control for the pilot-rated passenger in the right seat. Shortly after departing X50, he “heard a very loud boom” and the airplane “jolted nose down and to the left.” The controls were locked, and the pilot could not move the control column. The pilot applied force to the controls and they subsequently moved and felt freed from binding. He then decided to return to X50, and during a left turn onto the final approach leg of the traffic pattern, the controls intermittently locked two more times and the pilot performed a go-around.

During the next approach, when the pilot turned left onto base leg of the traffic pattern, he heard another boom, and the controls locked again. The airplane was descending rapidly, and the pilot performed another go-around. The pilot was able to fly the airplane back to the final approach leg of the traffic pattern without the controls locking; however, when the airplane was about 10 ft above the runway, the controls locked again. The airplane pitched nose down and to the left, the left wing contacted the ground, followed by the nose landing gear, which collapsed. The airplane then departed the left side of the runway, struck an embankment, and came to rest inverted in a pond.

Examination of the airplane revealed substantial damage to the engine mounts, firewall, and wings.

The airplane was retained for further examination.

EDGEWATER, Florida  — Crews from Edgewater Fire Rescue and neighboring agencies were dispatched to a reported plane crash Tuesday morning at 9:40 a.m., according to officials with the City of Edgewater.

There were reports of a small experimental aircraft upside down in a pond, investigators said.

Upon arrival, crews found two people had escaped, without injuries, from a plane that had slid off the runway at Massey Ranch Airpark and into a water-filled ditch.

Crews were still on the scene Tuesday morning to mitigate any hazards, and the cause of the incident will be under investigation by aviation authorities, officials said.

Analysis

The pilot completed a short cross-country flight and checked the wind conditions at the destination airport. He reported the automated weather station was out of service but received weather and runway information from an air traffic controller and airport personnel. The on-board weather information reported the wind was 10 to 12 kts at a 60°crosswind angle, so he set up for a normal landing. The pilot added that when he was about 30 to 50 ft agl, the airplane was hit with a gust of wind. He pulled back on the controls and stalled the airplane. The airplane bounced onto the runway, so the pilot elected to do a go-around. He applied throttle; however, the airplane veered left and impacted a drainage ditch. During the accident, the nose landing gear collapsed, and the right wing sustained substantial damage. About the time of the accident, the automated weather reporting station located 23 miles northwest of the accident site reported, wind from 290° at 10 kts gusting to 21 kts.
