On May 29, 2021, about 1100 mountain daylight time, a Maule M-5-235C, N9254E, was damaged when it was involved in an accident near Stanley, Idaho. The pilot and passenger were not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91
flight.

The pilot reported that the airplanes takeoff and climb from Johnson Creek Airport (3U2), about 30 minutes prior to the accident, was uneventful. Having reached an altitude of about 10,000 ft mean sea level (msl), he reduced engine power and initiated a descent into Thomas Creek Airport (2U8), with the intention of performing a low approach to check the runway surface, and then landing if conditions allowed.

About 500 ft short of the runway threshold the pilot applied full engine throttle, and the engine responded, but then began to progressively lose power. He pulled the carburetor heat control, confirmed the engine mixture was set correctly, and began to turn the airplane away from rising terrain. The airplane would not climb, and while he maneuvered it through a neighboring canyon, the engine began to regain power. He continued to assess the surrounding terrain and decided that there was not sufficient space to safely turn the airplane around. He then observed a flat area and attempted to maneuver the airplane to that spot and perform a landing at the lowest possible airspeed. During the landing flare, the airplane struck a log, and nosed over.

On May 22, 2021, about 1830 central daylight time, a de Havilland DHC-2, N218RD, sustained minor damage when it was involved in an accident near Oak Island, Minnesota. The pilot and one passenger were not injured, and one passenger sustained serious injuries. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.
  
According to local authorities who spoke with the pilot after the accident, prior to the flight, the pilot added hydraulic fluid to the reservoir due to a known leak in the amphibious float landing gear system. At some point during the flight, the left rear wheel extended and was protruding from the float. The pilot was unaware of the wheel position and attempted to land on the water near his residence. During the water landing, the wheel contacted the water, and the airplane’s nose immediately went into the water. The pilot and passengers exited the airplane, and the airplane subsequently flipped over.

The airplane was recovered to the shoreline, and the pilot noted the inside of the left float near the rear wheel was covered in hydraulic fluid. The pilot stated that an o-ring in the landing gear cylinder had failed.

The left wing tip contacted the ground during the go around and the airplane subsequently nosed over.

On May 30, 2021, about 0955 central daylight time, a Piper J4A airplane, N8501Z, sustained substantial damage when it was involved in an accident near Omaha, Nebraska. The pilot sustained minor injuries. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

During the preflight, the pilot reported that he filled the main fuel tank to the top and the auxiliary fuel tank had four gallons of fuel inside. The pilot planned on departing from the North Omaha Airport (3NO), Omaha, Nebraska, for flight operations in the airport’s traffic pattern. Before the takeoff, the fuel selector was positioned for the main fuel tank. During the takeoff, the airplane traveled about three quarters down the runway and climbed to about 300 ft above ground level. The engine sputtered once and stopped producing power. The pilot switched the fuel selector to the auxiliary tank and unsuccessfully tried to restart the engine.

The pilot maneuvered the airplane for a forced landing and the airplane came to rest in a heavily wooded area. The pilot reported that fuel was pouring in the cockpit after the airplane came to rest and he quickly egressed from the cockpit. The airplane sustained substantial damage to both wings and the fuselage. The wreckage was recovered from the accident site for a future examination of the airframe, engine, and propeller.

Analysis

The pilot receiving instruction reported that, during the first landing roll of the instructional flight, the airplane deviated slightly to the right and away from the runway centerline. The pilot receiving instruction corrected to the left and described the subsequent movement of the airplane as crabbing to the left while tracking down the runway. The flight instructor asserted that the pilot receiving instruction overcorrected with excessive left rudder and brake. The flight instructor told the pilot receiving instruction to get off the brake, and she applied right rudder. The instructor stated that the pilot receiving instruction “froze up” and she could not regain directional control of the airplane. The airplane exited the left side of the runway, nosed over and came to rest inverted. The airplane sustained substantial damage to the right-wing lift strut and the empennage.

The flight instructor and the pilot receiving instruction both reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation. At the time of the accident, the pilot receiving instruction was landing the airplane on runway 34 with variable wind at 5 knots, gusting to 11 knots.

On May 30, 2021, about 1720 Pacific daylight time, a Piper PA-22-150, N7159B, was substantially damaged when it was involved in an accident near Cashmere, Washington. The pilot sustained minor injuries. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

The pilot stated that he purchased the airplane about one month prior to the accident. He had purchased the airplane from a personal estate, and it had not undergone recent maintenance before the purchase. He performed a pre-buy inspection and hired a mechanic to do an annual inspection, bringing it to airworthy condition. Following the maintenance, the pilot checked the fuel and verified the throttle rigging (manipulation of the throttle control in the cockpit moved the cable from stop to stop on the carburetor). Both wing tanks contained fuel which he estimated to be half-full on both sides; he positioned the fuel selector to the right tank where it remained the entire flight.

The pilot stated that after a normal start he completed various taxi checks including a high-speed taxi with full engine power. The pilot then began a takeoff from runway 07 and applied full takeoff power (about 75% throttle). After reaching about 75 mph, the pilot rotated the airplane and it began to climb. After reaching about 250-300 ft agl, the engine power smoothly reduced to idle and in response, the pilot lowered the nose; the pilot verified that the throttle control in the cockpit remained at the takeoff power setting. The engine power temporarily increased back to full power for several seconds and subsequently quit. The pilot maneuvered the airplane away from the town ahead and touchdown in a marshy area, coming to rest inverted.

The pilot added that he attempted to troubleshoot the engine failure by verifying the fuel selector was on the right tank and that both magnetos were selected.
