Analysis

The pilot reported that, following 2 aborted landings, he attempted a third landing. Shortly after the initial touch down, the airplane encountered a wind gust, which lifted the left wing. The pilot applied left aileron input and initiated a go-around; however, the right wing struck the runway, which rotated the airplane to the right. Subsequently, the airplane exited the runway surface, impacted a ditch, and nosed over, resulting in substantial damage to the left wing and fuselage. The pilot reported that there were no preaccident mechanical malfunctions or failures with the airplane that would have precluded normal operation. The pilot reported at the time of the accident, he was landing on runway 30, and the wind was from 240° at 8 knots, gusting to 17 knots.

Analysis

The pilot planned to complete two takeoffs and landings (touch-and-goes) followed by a full stop landing. He executed a go-around and a touch-and-go before the accident landing. After the airplane touched down during the accident landing, the airplane tail began to settle to the runway. The pilot responded by pulling the control stick aft. About 2-3 seconds later, the airplane began to pitch up and bank to the right. At that point, the pilot “realized that [he] was behind the aircraft.” He attempted to execute a go-around but lost control of the airplane, resulting in a runway excursion. The airplane came to rest in a ditch off the right side of the runway. The right wing sustained substantial damage. The pilot reported that there were no mechanical malfunctions or failures with the airplane before the accident.

On July 1, 2022, about 1536 Pacific daylight time, a Beechcraft H35 Bonanza, N5546D, was substantially damaged when it was involved in an accident near LaCrosse, Washington. The pilot was seriously injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

The pilot reported that during a cross-country flight from Arlington Municipal Airport (KAWO), Arlington, Washington to Port of Whitman Business Air Center Airport (S94), Colfax, Washington, about 10 minutes from the destination airport, while on approach and as he performed his pre-landing checklist, the engine lost all power. The pilot reported that it sounded as if the engine went to idle power.  He then switched from the right to the left fuel tank, applied full throttle, increased the mixture to full rich, but the engine did not start. Shortly after, the pilot initiated a forced landing to a wheat field.

Subsequently, the engine separated from the firewall and the airplane sustained substantial damage to the engine mount and truss.

A local law enforcement officer reported that fuel was observed in the wing tanks.

The airplane and engine were recovered to secure facilities for further examination.

LACROSSE –   Kevin Kennedy, 65, of Arlington, Washington, was flying from Arlington to Colfax when his single-engine plane lost power and crashed north of State Route 26 near LaCrosse.

According to a Washington State Patrol news release sent out Thursday, Trooper Travis Brawdy located the plane with the assistance of directions given to him through WSP communications by the pilot.

Brawdy, after driving along a gravel road that was barely passable by vehicle, reached the pilot and began to immediately render aid. The pilot was trapped in the aircraft and required immediate medical attention. Life Flight was requested and transported the pilot to a Spokane hospital for medical care.

Analysis

After refueling the helicopter, the pilot entered into a light hover to return to his hangar. Due to obstacles on the left side of the helicopter, the pilot attempted to hover taxi horizontally to the right. While executing this maneuver, the right skid contacted the ramp and the helicopter entered into a dynamic rollover and came to rest on its right side, which resulted in substantial damage to the main rotor. On July 3, 2022, about 0930 eastern daylight time, a Cub Crafters CC11-160 airplane, N55JV, was substantially damaged when it was involved in an accident near Findlay, Ohio. The commercial pilot was not injured. The airplane was operated by the pilot as a flight test conducted under the provisions of Title 14 Code of Federal Regulations Part 91.

The pilot reported that the airplane had been previously involved in an accident in 2020 (in which he was not the pilot). He purchased the airplane and made extensive repairs to return it to an airworthy condition. This accident flight was the first test flight since the previous accident. The pilot taxied from his property to an adjacent off-airport soybean field. He performed a run-up without issue and operated the electric horizontal trim to a full nose up position as indicated on the primary flight display (PFD), and then moved the trim back down to the takeoff position.

When the trim indicator reached the takeoff position, he initiated the takeoff roll. The airplane accelerated normally, and he moved the control stick aft to rotate, however the control pressure felt “really heavy.” He reported that given that this was the first test flight after the previous accident, he immediately aborted the takeoff. In his effort to slow the airplane, it subsequently nosed over “slowly.” The left wing and empennage sustained substantial damage.

A Federal Aviation Administration inspector examined the airplane after it had been moved from the accident site. The horizontal trim position found at the flight control surface and as indicated on the PFD was consistent with a full nose down setting.

The airplane was retained for further examination.

Analysis

The pilot reported that while taking off from a downhill sloped meadow, the airplane encountered a strong quartering tailwind gust, which stopped the airplane from climbing. The airplane subsequently sank, and the left wing struck the ground. The airplane’s left wing and fuselage sustained substantial damage. The pilot reported that there were no preaccident mechanical failures or malfunctions that would have precluded normal operation. The pilot reported that the accident could have been prevented if he had selected a better takeoff site with more options in case of unexpected adverse conditions.

Analysis

The pilot was practicing aerobatics (about 400- 600 ft above ground level) when a “large” bird impacted the leading edge of the vertical stabilizer, which resulted in substantial damage. The airplane rolled and yawed slightly after the impact, but the pilot was able to maintain control of the airplane and land immediately without incident.

Analysis

While on final approach, the pilot asked the air traffic controller for the wind conditions. The pilot reported that he was “worried” about the wind conditions, and he forgot to extend the landing gear during the before landing checks. The airplane landed gear-up and came to rest upright on the runway. The airplane sustained substantial damage to the lower fuselage.

The pilot reported that there were no preimpact mechanical malfunctions or failures that would have precluded normal operation.

Analysis

The flight instructor reported that they were conducting a simulated single-engine approach in the multiengine airplane when the airplane began to yaw to the left. In an effort to regain directional control, he took control of the airplane from the student, and increased power on the left engine while simultaneously decreasing power on the right engine. Although both engines were still operating, he did not apply full power to both engines. The airplane impacted the ground, resulting in substantial damage to the fuselage. The pilot did not report any mechanical malfunctions or anomalies that would have precluded normal operation.
