NTSB Identification: GAA16CA153
14 CFR Part 91: General Aviation
Accident occurred Friday, March 18, 2016 in Bolingbrook, IL
Probable Cause Approval Date: 06/01/2016
Aircraft: BARNES THOMAS A VANS, registration: N442TB
Injuries: 1 Serious.

NTSB investigators used data provided by various entities, including, but not limited to, the Federal Aviation Administration and/or the operator and did not travel in support of this investigation to prepare this aircraft accident report.

The pilot of the tailwheel-equipped airplane reported that during the takeoff roll the airplane abruptly turned about 25 degrees to the left. He reduced the power to idle, applied right rudder, but reported that he over corrected to the right. He then applied left rudder to correct for the over correction. The airplane veered off the runway to the left, down an embankment and nosed over, which resulted in substantial damage to the empennage.

According to the pilot there were no preimpact mechanical failures or malfunctions with the airframe or engine that would have precluded normal operation.

The National Transportation Safety Board determines the probable cause(s) of this accident as follows:

The pilot's failure to maintain direction control during the takeoff roll, which resulted in runway excursion, collision with terrain, and nose over.

A pilot survived with minor injuries after wind blew his small airplane down a hill in Bolingbrook on Friday, police said.

The plane was taxiing or landing at 4:15 p.m. at Clow International Airport when the wind blew it into soft dirt, causing the pilot to lose control, according to Bolingbrook police Lt. Carter Larry.

"Once it hit the soft dirt, it rolled down a hill. There is an embankment," Larry said. "Once it hit the bottom, it slipped, causing minor injuries to the pilot."

The pilot was transported to a nearby hospital. There was no fuel leak and no fire, Larry said.

"Everything else is being gathered as part of the investigation. Our investigations take several weeks to complete," she said.
