On December 5, 2012, about 1200 mountain standard time, a Robinson R-44 Raven II helicopter, Canadian registration C-FRSO, impacted terrain approximately 12 miles southwest of Slave Lake, Alberta, Canada. The pilot, the sole occupant, was fatally injured.

The Transportation Safety Board (TSB) of Canada is investigating the accident. As the State of Manufacture of the helicopter and engines, the NTSB has designated a U.S. accredited representative to assist the TSB in its investigation.

Transportation Safety Board of Canada Statement:    RCC Trenton reported a 406 ELT registered to Gemini helicopters was active. Investigation revealed the pilot of a private Robinson R44 helicopter texted his wife he was departing Slave Lake for Edmonton at 1109 local time and his SPOT beacon showed an update about 20 kms from his take-off position at 1119. No updates occurred after that time. A 435 Sqn Hercules and 417 Sqn Griffon were tasked; a police helicopter from Slave Lake proceeded to the scene as well as a ground crew. The aircraft was located and the pilot was deceased; RCMP were on scene. TSB Edmonton will deploy two investigators.

 A 38-year-old man from Okotoks, Alta. has died after a helicopter crash near Slave Lake.

A 38-year-old man is dead after a helicopter collision near Slave Lake. 

Slave Lake RCMP said they were notified of an emergency beacon activated by a helicopter in the area at approximately 12:30 p.m. on Wednesday.

The downed helicopter, a Robinson R-44II, was located in a heavily bushed hillside area approximately 12 kilometres southwest of the Town of Slave Lake.

The pilot, a 38-year-old man from Okotoks, was the lone occupant of the helicopter and was found dead in the wreckage.

Investigators have determined the helicopter was flying from Peace River to Calgary.

The cause of the collision is under investigation.

RCMP say two helicopter companies in the Slave Lake area, along with the Canadian Forces Base in Trenton and Cold Lake, and the Slave Lake ESRD were all instrumental in responding and assisting in the crash.

Police say Glen Freeland, 38, died in the crash.
