On November 26, 2018, a Boeing 767-300 airplane, operated by Ethiopian Airlines, experienced a right engine (No. 2) fan blade failure during takeoff from the Addis Ababa Bole International Airport (ADD), Ethiopia. The airplane returned to ADD where an uneventful landing was made; there were no reported injuries to any of the persons on board. Examination of the airplane revealed two fan blades fractured at mid-span, inlet and fan cowl damage, and minor impact damage of the fuselage.

The investigation is being conducted by the Federal Democratic Republic of Ethiopia, Ministry of Transport Aircraft Accident Investigation Bureau. The NTSB is assisting as the state of manufacturer of the airplane and the engines.
