Analysis

The pilot reported that, he departed normally to fly the airport traffic pattern. After he turned to the base leg of the traffic pattern, he observed a low voltage indication on the annunciator panel and was unable to announce his base turn on the radio. The pilot then turned to the final approach leg of the traffic pattern without making the announcement on the radio and added three notches of flaps. He experienced “windshear” near the runway threshold and the airplane briefly ascended. The pilot attempted to regain his glide path by extending full flaps and the airplane floated down the runway. At the time he was concerned about climbing out, so he elected to continue the landing instead of performing a go-around. After the airplane eventually touched down on the runway, the pilot applied full braking, but the airplane skidded off the runway edge into a ditch, which resulted in substantial damage to the right wing.

According to the pilot, he misjudged the landing distance and landed long to salvage the landing instead of attempting a go-around. He further remarked that the electrical issue affected his radio and prevented him from announcing his base and final turns in the airport traffic pattern, which may have influenced his decision to land promptly. The pilot reported that there were no preaccident mechanical malfunctions or failures with the airplane that would have precluded normal operation.

The pilot’s failure to attain a proper touchdown point on the runway while landing in gusting wind conditions, which resulted in a runway overrun. Contributing to the accident was the pilot’s distraction due to an electrical issue.

Analysis

The pilot and flight instructor were conducting a flight review when on the downwind leg of the airport traffic pattern the pilot reduced the throttle to idle to simulate an engine failure, but then inadvertently extended the flaps instead of the landing gear. While on short final, the pilot increased engine power to ensure that the airplane reached the runway and, as such, the landing gear horn did not sound. However, when engine power was subsequently reduced to idle before touch down, the pilot then heard the landing gear horn and the airplane landed on the runway with the landing gear retracted. The airplane came to rest on the runway and sustained substantial damage to the engine keel beam. The pilot and flight instructor both reported that there were no mechanical malfunctions or anomalies with the airplane that would have precluded normal operation.

On September 17, 2021, at 1030 eastern daylight time, a Hughes 369HS helicopter, N420AT, was substantially damaged when it was involved in an accident in Ash, North Carolina. The pilot was not injured. The helicopter was operated as a Title 14 Code of Federal Regulations Part 137 aerial application flight.

According to the pilot, he flew from his home base to the remote job site and landed on his flatbed support truck to load the chemical hopper. The helicopter had about 25 gallons of fuel on board when it arrived at the truck. After landing, the pilot reduced the throttle to flight idle while the ground crew loaded 60 gallons of chemical to the hoppers. Prior to liftoff, the pilot increased the throttle until the engine and rotor rpm needles were in the green range of the tachometer. He believed that the throttle was in the flight detent. As he started to lift the helicopter from the truck, he noticed one of the truck’s doors was open, so he delayed liftoff until the ground crew could close the truck door. Subsequently, immediately after liftoff from the truck, the pilot heard the low-rotor rpm warning horn, and the helicopter began to descend. He attempted to pull more pitch; however, the helicopter did not respond.

The helicopter descended, impacted a field, and rolled over on to its right side. The pilot shut down the engine after the helicopter came to rest.

Examination of the helicopter by a Federal Aviation Administration inspector revealed that the helicopter sustained substantial damage to the tail boom. The tail boom was fractured just forward of the horizontal stabilizer. The flight controls were continuous from the cockpit controls to the main and tail rotor. All main rotor blades were fragmented and fracture separated near their roots.

The helicopter was retained for further examination.
