The pilot in the experimental amateur-built airplane reported that during his third touch-and-go landing, the airplane's airspeed was fast, and the airplane landed hard. The airplane's nose landing gear collapsed, and the airplane exited the right side of the runway. The airplane cartwheeled and came to rest upright.

The airplane sustained substantial damage to the right wing spar.

On September 19, 2018, about 1000 mountain daylight time, a Piper PA28 180 airplane, N8003W, impacted fencing and terrain during a forced landing near Pagosa Springs, Colorado, following an inflight loss of engine power. The airline transport pilot rated flight instructor and the student pilot were uninjured. The airplane sustained substantial damage during the impact with fencing. The airplane was registered to and operated by San Juan Flyers Inc. as a Title 14 Code of Federal Regulations Part 91 instructional flight. Day visual meteorological conditions prevailed in the area about the time of the accident, and the flight was not operated on a flight plan. The local flight originated from the Stevens Field Airport, near Pagosa Springs, Colorado, about 0900.

According to the flight instructor's accident report, the flight instructor was introducing engine failure emergency procedures to the student pilot. Emergency procedures were reviewed inflight, the instructor located a landing spot, and pulled the throttle to idle to simulate a loss of engine power. The student pilot conducted the emergency procedures on the left side of the cockpit that included switching the fuel tank selector position from right to left, verifying the primer was in and locked, turning on the fuel pump, checking magneto operation, and turning the carburetor heat on. The flight instructor subsequently added power to go around about 600 ft above the ground. The engine sputtered. Emergency procedures were reviewed again and the flight instructor noticed that the fuel pressure indication was zero. The flight instructor did not check the fuel selector nor could the selector be seen due to its location. The flight instructor subsequently performed a forced landing during which the airplane impacted fencing and ditches before coming to rest in a field. Following the accident, the flight instructor observed the fuel selector valve was about 1.5 to 2 inches between the left detent and off positions.

Subsequent to the accident, a Federal Aviation Administration inspector examined the accident airplane at the accident site. The airplane's engine was started, and the engine was operational.

The flight instructor reported no preimpact mechanical malfunctions or failures with the airframe and engine that would have precluded normal operation and had a safety recommendation to double check the fuel selector valve position during the forced landing demonstration.

The student pilot reported that he was performing touch-and-go's during a solo flight in the pattern. During the fifth landing, the airplane drifted over the runways left edge. He attempted to abort the landing by increasing power, but he did not apply enough right rudder. The airplane banked hard to the left prior to colliding with a fence. The airplane sustained substantial damage to both wings.

Analysis

The pilot reported that he initiated a takeoff roll on the 2,200-ft-long wet, turf runway, and that, about 1,650 ft down the runway, the airplane encountered standing water, and the speed decreased. He reported that he "still felt I had plenty of room to regain airspeed and take off safely." The airplane encountered standing water again on another section of the runway, and the speed decreased further. The pilot retracted two notches of flaps and attempted to climb the airplane, but it exited the departure end of the runway and entered a cornfield.

The airplane sustained substantial damage to the engine mounts.

The pilot reported that there were no mechanical malfunctions or failures with the airplane that would have precluded normal operation.

The pilot reported that she initiated the landing flare and the airplane floated about 100 feet and landed normally. About 400 feet into the landing roll, the airplane veered to the right, exited the right side of the runway and collided with taxiway lights prior to coming to rest.

The airplane sustained substantial damage to the lower fuselage intermediate bulkhead.

She reported that during landing the windsock indicated "no wind." However, after the accident she and her passenger witnessed dust devils close to the runway.

The airport's METAR reported that about the time of the accident, the wind was variable at 6 knots. The airplane landed on runway 30.

The pilot reported that there were no mechanical malfunctions or failures with the airplane that would have precluded normal operation.

On August 31, 2018, about 1345 eastern daylight time, a Cubcrafters CC11-160 airplane, N71BN, sustained substantial damage to the left-wing strut when it nosed-over during a forced landing to a soybean field following a complete loss of engine power. The airplane was descending to land at the Fairfield County Airport (LHQ), near Lancaster, Ohio. The pilot was not injured. The aircraft was registered to and operated by the pilot under the provisions of Title 14 Code of Federal Regulations Part 91 as personal flight. Visual meteorological conditions prevailed for the flight, which was not operated on a flight plan. The flight originated from The Duchy Airport (5NC5), Chapel Hill, North Carolina, about 1045, and was destined for LHQ.

The pilot reported that while descending from cruise flight to traffic pattern altitude, the airplane's engine began to run rough. The pilot applied carburetor heat which made the roughness very bad and he turned the carburetor heat off. When he did this, the engine lost all power. The airplane's altitude was about 2,000 ft. msl (1,200 ft. agl), and the pilot performed an off-airport landing to a farm field. During the landing, the airplane nosed over in the waist-high vegetation. The pilot reported that there were no mechanical malfunctions or failures of the airplane, and noted earlier use of carburetor heat at the beginning of the descent as an operator recommendation.

The weather conditions at LHQ about the time of the accident included a recorded temperature of 28° C and a dew point of 21° C. According to a carburetor icing probability chart, this was in the range of susceptibility for serious icing during descent power settings.
