MIAMI, Florida – A large presence of first responders was observed at Miami Executive Airport Sunday afternoon.

According to officials, a Piper aircraft could not deploy its landing gear and had one of its engines shut down.

The plane landed on the grass with no injuries reported, authorities said.

There were three people onboard the plane, including the pilot.

Authorities from U.S. Customs and Border Protection, Miami-Dade police and Miami-Dade Fire Rescue responded to provide assistance.

Officials said there were no reports of anything suspicious upon an initial investigation.

An Anchorage father and his 12-year-old daughter suffered minor injuries when their plane crashed near Point MacKenzie on Wednesday evening, officials said.

A group of people staying in a nearby cabin saw the aircraft crash just before 6 p.m., said Mark Ward, an investigator for the National Transportation Safety Board.

A good Samaritan pilot saw the plane, picked up the pilot and his daughter and brought them back to Anchorage, Ward said. They were treated at a hospital for minor injuries, according to Alaska State Troopers.

The plane sustained substantial damage, Ward said. The 57-year-old pilot planned to return to the area Thursday to photograph the wreckage and work on removing the plane, he said.

It wasn’t immediately clear what caused the plane to crash, Ward said. Investigators had limited details early Thursday but the investigator said he planned to talk with the pilot to get more information.

A small airplane had a rougher-than-expected landing at Byron Airport, Thursday, September 1 because of a mechanical malfunction.

When emergency crews arrived on the scene just after 6:43 p.m., they found a small, two-seat plane, nose down and leaking fuel. The pilot, who was the lone occupant of the aircraft, got himself out of the plane and was not injured, officials said. 

The Flight Design CTSW sustained damage to its right rear landing gear, propeller, and front landing gear. Fuel leaking from the right wing of the aircraft prompted a hazmat response for clean-up, according to officials on the scene. 

The pilot, who had just purchased the plane, said he was relieved to be walking away from the ordeal unharmed. 

The cause of the malfunction was not yet known by officials on the scene.

On September 4, 2022, at 0808 Pacific daylight time, a Bell 407 helicopter, N687AM, was substantially damaged when it was involved in an accident near Mount Baldy, California. The pilot was seriously injured. The helicopter was operated as a Title 14 Code of Federal Regulations Part 91 flight.

The helicopter was being used to shuttle water and equipment in support of a local charity event. The pilot had completed four shuttle flights and was approaching the main, off-airport landing zone to land when the low rotor RPM (revolutions per minute) warning sounded. He aborted the approach and attempted to increase power. The engine power did not recover, and the pilot performed an autorotation to a nearby road. The helicopter rolled upon landing, which resulted in substantial damage.

The helicopter was retained for examination.

A helicopter pilot was injured Sunday morning when his aircraft crashed on Mt. Baldy in San Bernardino County, authorities said.

The crash occurred around 8 a.m. in the area of Mount Baldy Road and Glendora Ridge Road.

The pilot was the only person on the helicopter. Paramedics from the Los Angeles County Fire Department responded to the crash site and treated him at the scene before transporting him to a hospital, the department said.

The extent of the pilot’s injuries was not available.

The cause of the crash is under investigation.

Mt. Baldy rises to an elevation of over 10,000-feet in the San Gabriel Mountains.

On September 4, 2022, about 0850 Pacific daylight time, a Beech BE-58 airplane, N142DR was substantially damaged when it was involved in an accident near Galt, California. The flight instructor and pilot receiving instruction were fatally injured. The airplane was operated as a Title 14, Code of Federal Regulations Part 91 instructional flight.

Multiple witnesses, who were hunting about one mile from the accident site, reported seeing the airplane spinning. One witness stated that “it was not nose down but more flat.” Another witness stated there was no engine noise.

Automatic dependent surveillance-broadcast (ADS-B) data provided by the Federal Aviation Administration (FAA) captured the airplane as it departed McClellan Airfield (MCC) Sacramento, California about 0707. The airplane the flew direct to Calaveras County Airport (CPU), San Andreas, California, at an altitude of 4,500 feet mean sea level (msl). The airplane arrived at CPU about 0726 and then departed to the northwest. The airplane climbed to an altitude of 3,000 ft, maintained about 180 knots, and arrived at Modesto City-County Airport (MOD), Modesto, California, about 0750. The airplane then proceeded to Tracy Municipal Airport (TCY), Tracy, California, and arrived about 0810. The airplane then departed TCY, climbed to an altitude of 4,500 ft, maintained about 180 knots, and proceeded to the north. About 0835, the airplane performed about a 360° turn to the right followed by a 360° turn to the left. After the two turns the airplane maintained 4,600 ft and continued to the northeast. About 0837, the airplane’s ground speed began to slow from 168 knots to 87 knots, and maintained about 4,600 ft, then increased its speed to 149 knots. About 0839, the airplane began to decelerate again. One minute later, while the airplane was decelerating through 100 knots, it began a left turn. The last ADS-B data points captured the airplane as it continued to decelerate and descend in a left turn. The last data point captured at 0841 recorded the airplane at an altitude of 400 feet msl (figure 1).

The airplane came to rest on a magnetic heading of 270°, in a narrow slough mostly covered with aquatic vegetation. There was no ground scar leading into the water, and only the vegetation immediately around the airplane appeared disturbed. All major components remained attached to the airplane except for the left propeller, which separated from the engine at the crankshaft flange. The one-piece windshield was shattered, and the cabin structure was displaced slightly over the left wing. The underside of the fuselage and both wings exhibited substantial upward crushing throughout. The empennage exhibited minor damage. Both engines remained attached to the wings and both propellers were feathered.

The airplane was recovered to a secure facility for further examination.

Two men, including a Raley's executive and the supermarket company's pilot, died when their Beechcraft 58 Baron plane crashed near Galt less than 20 minutes after taking off from Tracy Municipal Airport on Sunday.

The crash happened at an orchard near Christensen and Twin Cities roads in Galt, the Federal Aviation Administration said.

A report of the crash came in at 8:54 a.m., Sacramento County Sheriff's Office spokeswoman Sgt. Kionna Rowe said.

"The caller advised that they saw a small plane go down and heard a loud sound," Rowe said.

The Cosumnes Fire Department and deputies with the Sacramento County Sheriff's Office responded to the call, Rowe said. When authorities arrived at the scene, they located the aircraft in a "marshy area" inside of a private orchard.

Both victims, identified as 56-year-old Kenneth Mueller of El Dorado Hills and 68-year-old Richard Conte of Orangevale, were pronounced dead at the scene, according to Rowe.

Chelsea Minor, a spokeswoman for Raley's, said Mueller was the company's chief financial officer and Conte was its chief pilot.

"Our organization is deeply saddened and heartbroken," Minor said on Monday. "At this time, we are focused on supporting their families, friends and our colleagues."

The Federal Aviation Administration and the National Transportation Safety Board are investigating the cause of the crash.

Attempts to reach Tracy Municipal Airport for comment were unsuccessful.

Crews inspect a tour helicopter that made a precautionary landing Wednesday in a parking lot outside Aloha Stadium. The helicopter later took off and headed toward the airport. 



The helicopter that made an emergency landing Wednesday just outside Aloha Stadium is operated by the same tour company that’s faced scrutiny in recent years for multiple accidents and emergency incidents, including the 2019 Kailua crash that killed three people.

The chopper, which landed in the stadium’s lower Halawa parking lot around 2 p.m., belongs to Rainbow Helicopters, company officials confirmed. Police, fire and emergency medical crews all responded, but no injuries were reported, according to both Rainbow and Stadium spokeswoman Samantha Spain.

Rainbow representatives called the incident a “precautionary” landing.

“The pilot conducted the landing being conservative and made a normal touchdown in the parking lot,” John Cox, Rainbow’s vice president for safety, said in an email.

There was no damage to the helicopter, and the company is cooperating with an investigation into the matter and authorities, Cox said. He did not say how many people were aboard or give details on what prompted the landing.

“Safety is our number one priority and our pilot followed standard safety policies and protocols,” Cox added in his email.

Rainbow previously did business on Oahu as Novictor. The company largely flies Robinson R44 model helicopters — four-passenger helicopters that are generally smaller and less expensive than the turbine-engine models other Oahu operators use. The aircraft that landed at Aloha Stadium appears to be a different model, however.

Prior to Wednesday’s landing, the company had been involved in three crashes and one precautionary landing from 2018 to 2019, records show.

Those incidents included an emergency landing in Wahiawa, in which the helicopter encountered engine problems and had “substantial” damage, a crash landing at the Kaneohe sandbar in which the pilot blacked out twice and passengers had to take control of the aircraft, and the fatal crash in Kailua, where the helicopter broke apart amid heavy wind gusts and landed on a crowded residential street.

All three people aboard including the pilot were killed in that crash. No one on the ground was hurt.

Novictor was also named in early 2020 in a whistleblower report from a Federal Aviation Administration inspector that addressed multiple concerns with Hawaii’s air tour industry. That inspector, Joseph Monfort, alleged that FAA managers had “inappropriately” close ties with Novictor and had given the company special treatment.

At some point after the Kailua crash Novictor rebranded as Rainbow.

On Wednesday, the helicopter took off several hours after landing at Aloha Stadium and headed toward the Honolulu airport.

On September 2nd, Company 5 was alerted to assist Clearfield EMA with an aircraft emergency located at Clearfield Lawrence Airport (KFIG) in Clearfield, Pennsylvania.  A Cessna 172 Skyhawk went off the runway with no reported injuries to its operators. Crew members provided scene monitoring and cleanup until the Federal Aviation Administration arrived.

A autogyro aircraft went down Saturday morning a Brackett Field Airport in La Verne.

Images provided by the La Verne Fire Department showed the damaged aircraft on its side on what appeared to be one of the airport’s runways.

The plane was badly damaged with glass and parts strewn across the ground.

Only minor injuries were reported as a result of the crash, the Fire Department stated.

The airport was temporarily closed amid the investigation and cleanup of the incident.

HOLMES COUNTY, Ohio (WOIO) - Federal Aviation Administration (FAA) officials were called out to the Holmes County Airport Thursday afternoon after a wheel fell off a plane.

According to Holmes County Sheriff deputies, an 83-year-old pilot had just landed his Van's RV-8 aircraft and was taxing when a front wheel came off the plane.

This caused the landing gear to dig into the runway.

Deputies said the aircraft then spun around about 180 degrees before stopping on the south edge of the runway.

There was minor damage to the aircraft, but no injuries to the pilot or his co-pilot.

The airport will be re-opened after the scene is cleared.

Deputies said the pilot, Kingsley Owen, of Hanover, Pennsylvania, and his co-pilot, Stephen Robson, of the United Kingdom, were flying to Blakesburg, Iowa, for a fly-in. They had stopped at the airport for fuel.
