On July 9, 2018, about 1100 Pacific daylight time, a Grumman G-164B airplane, N6696K, experienced a total loss of engine power and the pilot executed a forced landing in a rice field near Colusa, California. The commercial pilot was not injured and the airplane sustained substantial damage to the upper wing and empennage. The airplane was registered to and operated by Valley Air Service Ltd as a 14 Code of Federal Regulations Part 137 aerial application flight. Visual meteorological conditions prevailed, and no flight plan was filed. The flight originated from the Colusa County Airport (O08), Colusa, at an unknown time.

A representative from the operator reported that the pilot was finished fertilizing a field when the airplane's engine started to run rough and lose complete power. The pilot attempted to land at a nearby private landing strip, however, the airplane landed short in a rice field and came to rest inverted.

The airplane has been recovered to a secure location for further examination.

The flight instructor reported that he asked the commercial pilot rated student to simulate a power-off emergency landing with a 180ยบ turn to the runway and directed her to "lower the gear when landing was assured." After initiating the power off approach, the student turned the airplane from the downwind leg onto the base leg of the traffic pattern and extended the landing gear. The sink rate increased, and she attempted to correct with full engine power, but the decent continued. The airplane landed short of the runway and struck a ground lighting system. She then initiated a go-around and landed without further incident.

The airplane sustained substantial damage to the right wing.

The instructor reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation.

The automated weather observation station located on the airport reported that, about the time of the accident, the wind was from 080° at 8 knots. The airplane landed on runway 09.

According to the pilot in the tailwheel-equipped airplane, he circled the airport to check the wind direction and speed, and then he initiated a final approach about 70 knots with 3 notches of flaps.

He reported that the airplane encountered a wind gust prior to initiating the landing flare. Upon touchdown of the wheel landing the airplane bounced and he applied forward yoke pressure.

The airplane began to drift to the right of the runway center line during the landing roll and he applied left rudder and left aileron to counter the drift, but when the tailwheel touched down, the airplane veered to the left.

He applied right rudder and right brake, but the right main landing gear collapsed, the left wing lifted, and the right wing struck the ground. The airplane exited the left side of the runway and it came to rest in the grass safety area.

The airplane sustained substantial damage to right wing spar.

The pilot reported that about the time of the accident, the wind was from the west-southwest, about 7 knots. The pilot landed on runway 32.

The METAR at the accident airport reported that about the time of the accident, the wind was calm, and the skies were clear.

The pilot reported that there were no mechanical malfunctions or failures with the airplane that would have precluded normal operation.

According to the helicopter pilot, he was performing a night agricultural flight. During takeoff from a platform located on top of the tender, his collective application was too aggressive, and during the ascent the engine sputtered. The engine RPM and main rotor RPM decreased, and he tried to regain the RPMs by lowering the collective.

The helicopter was moving forward and descending, so he applied aft cyclic to arrest the forward movement, and he attempted to land. The left skid touched down in the soft dirt and the pilot applied aft cyclic to prevent a nose over, but the main rotor blades struck the tailboom. The helicopter rolled forward, and the nose of the helicopter spun to the left before the helicopter came to rest on its left side.

The helicopter sustained substantial damage the fuselage, tail rotor drive system and skid tubes.

Additionally, the pilot asserted that the accident could have been prevented if he would not have been in a hurry to complete the operation.

The pilot reported that there were no mechanical malfunctions or failures with the helicopter that would have precluded normal operation.

RULO –No injuries were reported after a small helicopter crashed last night in extreme southeast Nebraska.

Shortly after 9:00 p.m., Richardson County sheriff’s deputies and state troopers responded to the scene just south of Rulo and one mile north of the Kansas border near the White Cloud Casino.

The helicopter could be seen laying on its side with debris scattered throughout a soybean field. The scene was secured by law enforcement as they awaited further instructions from the Federal Aviation Administration.

The identity of the pilot was not released by authorities pending the investigation.

The pilot of the tailwheel equipped airplane reported that, after a three-point landing, the airplane began to "shudder" and veered left. As the airspeed decreased, the tailwheel touched down again on the runway, and the airplane shuddered and veered left again. He corrected with opposite rudder, left brake and aileron, but the airplane ground looped to the left, and the main landing gears collapsed.

The airplane sustained substantial damage to the fuselage.

The pilot reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation.

The pilot added that, about one hour prior to the accident, the wind was from 340ยบ at 12 knots, gusting 17 knots. The automated weather observation station located on the airport reported that, about the time the accident, the wind was from 340° at 8 knots. The airplane landed on runway 5.

There were no reported injuries after a plane ran off the runway at the North Myrtle Beach airport Monday morning.


At about 11 a.m., emergency crews responded to the Grand Strand Airport for reports of a crash. North Myrtle Spokesman Pat Dowling said a biplane ran off the runway, possibly due to a landing gear problem.


The plane is used to fly tourists around the beach area. Two passengers and a pilot were on board when it went off the runway. There were no reported injuries.


Tina Farley lives near the airport and said she didn't hear a loud crash. She only noticed there was activity at the airport when fire crews passed. Other neighbors said they haven't seen a crash in years of living nearby. 

The red biplane sat on the runway with one of its wings tilted into a ditch as emergency crews worked the scene. A small crowd gathered along the fence of the airport after they heard reports of the crash.


Representatives for the plane company did not respond to requests for comment after the crash.
