Aviation Accident Preliminary Report - National Transportation Safety Board

Investigator In Charge (IIC): Boggs, Daniel

The National Transportation Safety Board did not travel to the scene of this accident.

Additional Participating Entity:
Federal Aviation Administration / Flight Standards District Office; Albany, New York
Location: Monticello, New York
Accident Number: ERA22LA317
Date and Time: July 15, 2022, 12:45 Local
Registration: N6195R
Aircraft: Cessna T210F
Injuries: 1 None
Flight Conducted Under: Part 91: General aviation - Personal

On July 15, 2022, at 1245 eastern daylight time, a Cessna T210F, N6195R, was destroyed when it was involved in an accident near Monticello, New York. The pilot was not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

The pilot stated that he conducted an uneventful flight earlier during the day from Maine to Sussex County Airport (FWN), Sussex, New Jersey, to fly a potential buyer in the airplane, which he was selling. After the short flight, the pilot dropped off the potential buyer at FWN and planned to fly to Sullivan County International Airport (MSV), Monticello, New York, for fuel prior to returning to Maine. Upon beginning a descent to MSV the engine began losing power and “making noises.” The pilot verified fuel quantity and mixture settings. He also estimated that 10 gallons of fuel remained in each tank. He switched fuel tanks, but the power loss continued. He subsequently began looking for a place to land the airplane.

The pilot then heard what sounded like an “explosion” from the engine. The oil service door blew open and began spewing oil and smoke from the service door opening, covering the windscreen. Smoke then began entering the cabin. The pilot reported selecting the flaps and landing gear down but did not have time to verify extension. He identified a field as an emergency landing location. As the airplane approached the field, about 200 ft above the ground, the pilot observed high voltage powerlines crossing the field and he repositioned the airplane for a landing on a road. He landed on the road but did not think the landing gear was down since the airplane did not have hydraulic power. The airplane caught on fire, and he egressed before the cockpit was consumed.

Post-accident examination of the airplane by a Federal Aviation Administration inspector revealed that landing gear was not extended, and that the engine had a large section of the crankcase missing from the forward left side. The No. 6 connecting rod was laying on top of the engine and the crankshaft and camshaft were fractured.

The wreckage was retained for further examination.

THOMPSON, New York — Emergency personnel responded to the scene of a small plane crash on State Route 42 in the Town of Thompson on Friday afternoon.

The pilot, 66 year-old Mark Simmons from Troy, Maine, walked away uninjured.

It was just before 1 p.m. when the single-engine aircraft went down approximately three miles south of the Village of Monticello and at the border with the Town of Forestburgh.

According to the Sullivan County Sheriff’s Office, Simmons was piloting his Cessna 210 from Sussex, New Jersey, to the Sullivan County Airport in Bethel when he experienced  engine trouble approximately nine miles out.

Simmons then managed to set the plane down on a stretch of State Route 42 that had minimal traffic at the time. He emerged from the aircraft by himself before the front end was consumed in flames.

Simmons was the only person onboard the aircraft.

The Monticello and Forestburgh Fire Departments responded to the scene, along with the Sullivan County Sheriff’s Office and later the Federal Aviation Administration (FAA). The Rock Hill Fire Department, Mobilemedic EMS and the New York State Department of Transportation assisted at the scene as well.

Route 42 was shut down in both directions from the crash site with traffic being diverted along back roads.

“Thank god no one was injured or killed,” said Sullivan County Sheriff Mike Schiff, a pilot himself.

The crash remains under investigation by the National Transportation Safety Board along with the FAA.

On July 17, 2022, about 2000 central daylight time, a Just Highlander airplane, N7514N, sustained substantial damage when it was involved in an accident near Boswell, Oklahoma. The pilot and student pilot sustained fatal injuries. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

According to the family members and friends of the two occupants, the purpose of the local area flight was to conduct low-level flight operations over and on the Red River. The Red River flows from the west to the east and separates Oklahoma and Texas. The experimental tailwheel airplane was equipped with tundra tires, that facilitates off-airport landings and takeoffs.

The airplane departed from the Jones Field Airport (F00), Bonham, Texas, around 1925. The pilot was seated in the left seat and the student pilot was seated in the right seat. The student pilot texted a family member at 1928, that the airplane was airborne. The expected return time back to F00 was undetermined.

After not hearing back from either occupant, concerned family members initiated a search and found the wreckage around 0200 on July 18, 2022. The airplane had come to rest, nose down with the empennage in a near vertical position, on a remote island in the Red River. The airplane sustained substantial damage to the fuselage and to both wings. The wreckage is pending recovery for a future examination.

A review of astronomical data showed that on the day of the accident, sunset was at 2035. The estimated density altitude for the accident site was 3,060 ft above msl.

Hadley “Had” Wade Garrett Workman

DURANT, Oklahoma - A plane crash in the Red River early Monday morning left two people dead.

Ryan Richardson and Hadley Workman were two young men with a passion for flying planes.

Richardson was the first to enroll in flight lessons with flight instructor Jacob Toews.

“He was so sharp on picking up things,” Toews recalls. “He could do anything. I mean, literally anything.”

Then along came Workman, who recently started taking flight lessons with his mother.

“He was a bright student as well. He had taken his last lesson with me last Saturday.”

But on Sunday night something went tragically wrong. The single engine plane Richardson was piloting went down in the Red River, killing both Richardson and Workman.

While the investigation is ongoing, Toews tries his best to make sense of it all.

“I figure they were probably doing landings on the sandbars in the river there, which is a really fun activity. So, I have no clue what happened because he was such a good pilot.”

As Toews mourns the loss of his two students, he will remember them as being innovative young pilots who were taken away too soon.

A pilot was taken to a hospital with serious injuries after a small plane made an emergency landing Monday morning in a wooded area in the Palatka area, authorities said.

According to the Florida Highway Patrol, a single-engine, experimental aircraft from the Putnam County area was departing from Palatka Municipal Airport, also known as Kay Larkin Field, on Reid Street, around 8 a.m. when the pilot — a 55-year-old man from Franklin, Tennessee — experienced mechanical failure and attempted to circle back to the airport but had to make an emergency landing.

The Putnam County Sheriff’s Office said the pilot called 911 around 8:10 a.m. to report he was having issues with his plane and was trying to return to the airport. But, deputies said, the call cut off, prompting a search of the area near the airport. Air assets, including a helicopter and another plane, were sent to look for the wreckage, but the tree canopy was too heavy, deputies said. The pilot was eventually able to use his cellphone to call 911 again, and first responders were able to ping his phone in the area of Browns Lane near the airport, find him and pull him from his plane shortly after 9:30 a.m., deputies said.

News4JAX was initially told that the pilot, the only occupant of the plane at the time of the crash, was flown, “as a precaution,” to HCA Florida Orange Park Hospital with various cuts and bruises. But a news release from FHP described the pilot’s injuries as serious.

The Highway Patrol is investigating, and the National Transportation Safety Board has been notified.

According to a witness, the pilot built the kit biplane in 2016 and equipped it with a Volkswagen engine. He stated that since 2016, the pilot had operated the airplane about 30 hours. During that time there were several problems with the engine operating correctly and oil leaks. In 2020, the pilot experienced a hard landing and fractured the main landing gear. During the next 2 years, the pilot repaired the airplane and installed a Lycoming O-320 engine on the airframe. The work was completed in April 2022. Several slow speed taxies were performed over the next few weeks and on the day of the accident, the pilot intended to fly the airplane for the first time. The witness met the pilot at 0900 to help him with the preflight inspection and pull the airplane out of the hangar.

The witness added that there were no issues with the preflight inspection and the 12 gallon capacity fuel tank was full. The pilot stated that he planned to climb straight-out to about 500 ft above ground level (agl), then turn left and stay close to the runway while he checked the airplane. The pilot started the engine and warmed it up for about 10 minutes. He then gave the witness a “thumbs-up” and applied full throttle. During the takeoff roll, the airplane veered to the left and the witness thought the airplane was going to hit a large round hay bale next to runway. The airplane lifted off the ground and cleared the hay bale by 6 ft. The pilot flew back toward the runway centerline to maintain runway heading but was going “very slow.” After the airplane cleared the treetops, the pilot started a slow left turn about 200 ft agl.

The witness observed the tail drop down and the left wing roll hard over. The airplane then spun to the ground.

The accident site was located in a field about 1,600 ft east of the runway. The airplane impacted in a nose down attitude. The wooden propeller blades were splintered into numerous pieces at the impact point. The main landing gear separated, and the airplane slid about 60 ft before it came to rest upright on a 15° magnetic heading.

The airplane was completely consumed by fire. The engine was fractured off at the mounts and located under the left wing. The fabric was consumed by fire except for a small piece on the upper and lower right-wing tips. Flight control continuity was established from the control surfaces to the joystick which was thermally destroyed.

Examination of the airplane with a representative of the kit manufacturer revealed that the left outer interplane strut was not correctly attached to the left lower wing compression strut.

The bolt and nut were present and through the outer interplane strut but were not attached to the lower wing compression strut.

Ret. U.S. Army Maj. Gary L. Knight, 68, of Lansing, Kansas, died Monday, July 18, 2022. He was born Feb. 18, 1954, in Chicago Heights, Illinois, to Paul and Dessie (Kivett) Knight.

Gary attended the Virginia Military Institute on a four-year scholarship and graduated in 1976 with military honors. He also graduated from Army Airborne and Ranger schools. Gary was commissioned in the U.S. Army Armor Branch after graduating from VMI and retired as a U.S. Army Armor Branch Major. He then began a second career as a pilot and worked his way up to flying commercial passenger planes.

On Jan. 30, 1988, Gary married Joanne McGrath in Kenmore, N.Y. She survives at the home. He is also survived by two children, a daughter, Amanda Boyers, of Kansas City, Mo., and son, Colin Knight, of Chandler, Ariz. Two sisters, Sheila Fiorini, of Crete, Ill., and Shae Knight of Oglesby, Ill.; three brothers, Glen Knight of San Antonio, Texas, Paul Knight of Wheeler, Ore., and Evan Knight of Medford, Ore.; three granddaughters, Adeline Boyers, Phoebe Boyers and Séraphine Boyers; friends from his time attending Virginia Military Institute, Kurt Luckenbill, Charles Archer and Ron LaGrone; the “Colonel” of Gary’s flying crew from the airport, Jeff Givens; and his lifelong friend, Doug Johnson.

Gary enjoyed nightly dinners at home with his wife and listening to the Retro Cocktail Hour on the radio. Having a shared sense of adventure and drive, Gary loved supporting his daughter in her many endeavors and hobbies too. He also loved anything history related, but particularly enjoyed reading about and discussing military and space-related historical events. He enjoyed frequent conversations with his son about their shared military experiences. Building his replica of a WWII Nieuport 28 was his way of combining two of his strongest passions in life, history and flying.

Visitation will be from 2 to 4 p.m., Sunday, July 31, 2022, at the R.L. Leintz Funeral Home. A funeral service will begin at 10 a.m. Monday, August 1, 2022, at the funeral home. A pilot and his passenger were uninjured Monday morning after an emergency forced a sudden landing on a highway moments after taking off from the Granbury Regional Airport.

According to the Texas Department of Public Safety, the pilot of a single-engine, fixed-wing aircraft was departing the airport when the plane lost power.

The pilot, identified as 67-year-old James Winston Jr., of Tolar, brought the plane down on Pearl Street/Business 377, just south of the airport and west of downtown Granbury. When the plane touched down the gear failed and the aircraft came to rest on its belly about 1,100 feet from runway 32.

The plane had visible damage to the gears, wings and prop. It's not clear what caused the plane to lose power.

There was one passenger aboard the aircraft, 26-year-old Justin Winston, of Tolar. Both men were uninjured, officials said.

Federal Aviation Administration records show the Rockwell Commander 112 is owned by the pilot.

The Federal Administration Administration will be investigating the incident.
