Analysis
  
The commercial pilot reported that, while spraying a field during his first agricultural application flight, he flew the airplane over the field east and west and was blinded by the sun. For the next application, he decided to fly north and south over the field, so the sun was not in his eyes. The airplane was operating normally, and he finished spraying the field. He remembered that he was supposed to spray another field to the east, but he could not remember anything after that. The airplane was found resting in the field. The airplane and a 150-ft-tall antenna tower located on the east edge of the field were substantially damaged, and a portion of the tower structure was found entangled with the right wing. The pilot was seriously injured. Review of astronomical data indicated that the sun at the time of the accident in relation to the tower location was almost due east. The pilot reported that there were no preaccident mechanical malfunctions or failures with the airplane that would have precluded normal operation.

Analysis

The pilot reported to a Federal Aviation Administration inspector that about 5 miles from the airport the airplane engine began to have trouble, so he turned on the electric fuel pump. He switched fuel tanks, but the engine still lost power. The pilot was unable to make it to the airport, so he conducted a forced landing to a marsh/swamp. Due to the accident location, an on-scene examination of the airplane was not conducted, and the airplane was not recovered.

The pilot did not return a completed National Transportation Safety Board Pilot/Operator Aircraft Accident/Incident Report Form 6120.1 as requested.

On September 3, 2020, about 0715 central daylight time, a Cessna 340 airplane, N5178J, was substantially damaged when it was involved in an accident near Oklahoma City, Oklahoma. The pilot was not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

The pilot said he was flying from Sundance Airport (HSD), Oklahoma, to Burnet Municipal Airport-Kate Craddock Field (BMQ), Texas, at 18,000 ft, when the right windscreen blew out 50 miles southwest of Oklahoma City. The pilot returned to HSD and landed uneventfully. The windshield is considered part of the structure of a pressurized aircraft, therefore the airplane was substantially damaged. The airplane was retained for further examination.

Analysis

According to the right seat pilot/owner of the tailwheel-equipped airplane, she was demonstrating a short field takeoff to the passenger on a grass area adjacent to the airport’s single paved runway. After adding power and releasing the brakes, the airplane veered to the left toward a ditch between the grass area and the paved runway. The pilot attempted to steer away from the ditch by applying rudder control inputs but was unsuccessful. As the airplane approached the ditch she elected to rotate. The airplane briefly became airborne then stalled and impacted terrain. The pilot and the passenger were seriously injured and the airplane’s fuselage and both wings were substantially damaged. The pilot reported that there were no preimpact mechanical failures or malfunctions with the airplane that would have precluded normal operation.

On August 26, 2020, about 0815 Pacific daylight time, a Cessna 210, N9540T, was substantially damaged when it was involved in an accident near Chino, California. The pilot was not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

The pilot reported that the accident flight was the first flight since the airplane had undergone landing gear maintenance. While in the airport traffic pattern, he extended the landing gear, however, noticed that it would not lock into the down position. Despite multiple attempts to cycle the landing gear, which included utilizing the emergency landing gear extension procedures, the pilot was unable to obtain a positive down and locked indication. The pilot stated that during landing on runway 26R, the main landing gear collapsed, and the airplane came to rest upright on the runway.

Postaccident examination of the airplane by a Federal Aviation Administration inspector revealed that the horizontal stabilizer and elevator were substantially damaged. The wreckage was recovered for further examination.
