On July 3, 2020, about 1700 central daylight time, an Air Tractor AT-602 airplane, N602NY, was substantially damaged when it was involved in an accident near Weldon, Arkansas. The pilot was not injured. The airplane was operated as a Title 14 Code of Federal Regulations (CFR) Part 137 agricultural flight.

The pilot stated that he departed a private airstrip near Weldon, Arkansas, about 1650 with about 75 gallons of Jet-A fuel and a load of dry granular fertilizer. The pilot reported that the aerial-application portion of the flight was uneventful; however, as he was returning to base the airplane had a total loss of engine power about 3 miles from the airstrip. The pilot stated that the airplane was established in cruise flight about 400 ft above ground level when the engine "spooled down." The pilot stated that he was unable to restore engine power by advancing the power and propeller levers to full forward and activating the fuel boost pump and ignitors. The pilot did not recall looking at the cockpit engine gauges after the loss of engine power. The pilot stated that he lost directional control of the airplane after he made a firm landing in an open field. The pilot estimated that the flight was about 10 minutes in duration. The aft fuselage sustained substantial damage when the tailwheel separated during the ground loop after landing.

On July 6, 2020, before he notified the National Transportation Safety Board (NTSB) or Federal Aviation Administration (FAA) of the accident, the pilot had his aviation mechanic examine the airplane at the accident site to begin a repair estimate. The mechanic stated that there was ample fuel in the fuel tanks and that he was able to start up and run the engine at the accident site. The mechanic noted that the engine responded to his power lever inputs, his movement of the propeller lever cycled the propeller without any anomalies, and that the engine could generate 1,500 propeller rpm. The mechanic stated that all observed engine parameters were nominal during the engine test run at the accident site.

The airplane was partially disassembled and recovered from the field to a repair station for repairs and examinations. The recovery crew reported that about 56 gallons of fuel was drained from the wing tanks, and that the recovered fuel was evenly distributed between the wing fuel tanks. A FAA inspector reported that the fuel lines, fuel tank inlet screens, and fuel filters were free of any blockage, water, or significant particles. The airplane's digital fuel management system indicated that 50.3 gallons of fuel was remaining after the postaccident engine test run at the accident site.

The entire airplane was retained for additional examination and testing after multiple airframe repairs have been completed. The engine, a Pratt & Whitney PT6A-65AG, serial number PCE-PN0209, had accumulated 1,521.6 hours since new. The engine had accumulated 141 hours since it was last inspected on May 1, 2020.

On May 27, 2021, about 1835 eastern daylight time, an experimental, amateur-built Early Bird Jenny, N831HC, was destroyed when it was involved in an accident near Stoneville, North Carolina. The private pilot was fatally injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

The co-owner/builder of the airplane stated that he flew the airplane the morning of the accident, returning to the airport after about 30 minutes due to rain. He and the accident pilot, who was the other owner/builder of the airplane, moved the airplane into the hangar, where they waited for the rain to move through the area. The co-owner reported that his flight was uneventful and that the airplane
performed well and handled as expected, though the takeoff profile was “flatter than usual” due to the warmer temperature that day. Later that afternoon, the accident pilot departed on his flight.

The co-owner reported that the pilot’s taxi to runway 31 was normal, and that the engine was producing power, but that the takeoff did not “look right.” Both he and another witness to the accident reported that the airplane was climbing in a nose-high attitude and appeared slow before the wings began to rock back and forth. About 150 ft above the ground, the right wing dropped, the airplane impacted terrain next to the runway, and a post-impact fire ensued.

The accident site was located about 175 ft north of the runway 31/13 centerline and about 2,500 ft from the runway 31 threshold. The airplane came to rest on a southerly heading and was destroyed by the post-impact fire. All major components of the airplane were accounted for at the site, and flight control continuity was established from the flight control surfaces to the cockpit area. The wooden propeller was highly fragmented and separated from the engine. The engine displayed extensive thermal damage. The crankshaft could not be rotated by hand, but valve train continuity was displayed upon rotation of the camshaft. The throttle control cable was continuous from the carburetor to the cockpit area.

Reported weather conditions at the airport around the time of the accident included calm wind, temperature 27°C, dew point 18°C, and an altimeter setting of 29.97 inHg. Calculated density altitude was about 2,446 ft.

ROCKINGHAM COUNTY, North Carolina — A pilot has died in a small plane crash at Shiloh Airport in Rockingham County.

The Rockingham County Sheriff's Office said the plane crashed Thursday evening just before 7 p.m. at Shiloh Airport on Settles Bridge Road in Stoneville. 

Investigators said the plane caught on fire in the crash.

The Rockingham County Sheriff's Office said they will not be releasing the name of the pilot at this time.

"We're sorry for their loss and it certainly is tragic for the individual and families involved. Our thoughts and prayers go out to the family," Rodney Cates said with Rockingham County Emergency Services. 

The Federal Aviation Administration was contacted and began its investigation Friday morning. The National Transportation Safety Board is leading the investigation into what caused the crash.

"We spoke with one witness earlier this afternoon who said he saw the airplane taking off. It reached an altitude of about 100 feet or so and then entered a turn and then impacted terrain," said Allison Diaz of the NTSB.

Investigators have not released the identity of the pilot. However, they identified the plane as an Early Bird Jenny, which is an amateur-built replica of a World War 1 plane.

Airport staff said the pilot who co-owned the plane was a regular at the airport and a plane enthusiast. They said he had just finished building the plane there 3 months ago and was flying locally to get the required flight hours for the certification process. No information was readily available about how many flight hours the plane had accumulated before the crash.

"Will be looking at the engine, looking for maintenance records on the airplane as well as delving into the pilot's experience and qualifications," Diaz said.

Investigators said they'll also look to see if the weather conditions at the time of the crash played any role.

The NTSB said a preliminary report on the crash would be available in two weeks while the full investigative report could take 12 to 18 months.

On May 27, 2021, about 0750 central daylight time, a Grumman G-164B airplane, N8376K, was substantially damaged when it was involved in an accident near Celina, Ohio. The pilot sustained serious injuries. The airplane was operated as a Title 14 Code of Federal Regulations Part 137 aerialapplication flight.

The pilot stated that he departed on the local flight with about 80 gallons of Jet-A aviation fuel and a 240-gallon mixture of fungicide and insecticide. The winds were light out of the east with a few clouds 3,000-4,000 ft above ground level. The pilot stated that shortly after takeoff from runway 8 he felt the engine lose thrust, but the engine speed remained at 100% according to the airplane’s cockpit indicator. The pilot established best glide speed and began a slight right turn to avoid trees. He then jettisoned the chemical load in attempt to maintain altitude, but the airplane descended into a waterway about 200 yards south of the departure end of runway 8.

An examination of the propeller revealed it remained attached to the engine propeller shaft flange, and all three blades remained attached to the propeller assembly. All three blades exhibited leading edge gouging, chordwise/rotational scoring on both camber and face sides, bending aft and twisting towards low pitch (Figure 1). All three blades had 3-5 inches of the tip fractured/separated. All observed damage was consistent with impact while the propeller was rotating under power and with high impact forces.



An examination of the engine revealed the control linkage to the propeller pitch control (PPC) was not connected. The associated bracket was not attached to the splined shaft of the PPC, the retaining bolt was loose, and the bolt was not secured by safety wire (Figure 2). The engine was retained for additional examination at the manufacturer.

On May 26, 2021, about 1430 pacific daylight time, a Boeing A75N1, N75016, was substantially damaged when it was involved in an accident near Frazier Lake Airpark (1C9), Hollister, California. The pilot and pilot-rated passenger sustained minor injuries. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

The pilot reported that, after he purchased the accident airplane, he sent the airplane to a local maintenance shop for an annual inspection. A couple of days later, the airplane was returned to service and the pilot received about a three-hour walkaround and preflight covering the details of the airplane.

On the day of the accident, the pilot and a pilot-rated passenger completed a preflight inspection and engine run up with no anomalies noted. They performed 10 full stop takeoff and landings, then flew to Hollister, California to refuel the airplane. The pilot performed another takeoff and landing when they returned to 1C9. During the initial climb out, the airplane stopped gaining altitude. The passenger reported that he noticed that the airplane was not gaining nor losing altitude, so he verified that the propeller lever, the throttle lever, the mixture control lever, and the fuel selector lever, were all in their correct positions. The pilot added that although the “RPMs were just dropping,” the airplane never lost all power. Shortly after, the pilot initiated a right turn to maneuver away from telephone lines and completed a forced landing in a soft dirt field, where airplane came to rest upright.

The airplane and the engine were recovered and will be examined.

On June 16, 2022, about 0830 central daylight time, a Meyers 200A airplane, N489C, was substantially damage when it was involved in an accident near Mid Valley Airport (TXW), Weslaco, Texas. The pilot was not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

The pilot stated that he intended to complete a cross country flight to Florida and had about 65 gallons of fuel onboard. After takeoff from TXW, he entered the left traffic pattern to overfly the runway before departing to the northeast. While on the downwind leg, the engine experienced a total loss of power and the propeller wind-milled. As he turned toward the airport, he estimated that he had the runway made so he extended the landing gear. The landing gear provided too much drag and he was unable to make the runway, so the airplane descended into an area of trees. The airplane collided with trees and came to rest vertically with the tail resting on a tree (Figure 1). Witnesses converged on the accident site and assisted the pilot in egressing from the airplane.

The engine was mostly separated from the firewall and the propeller separated from the engine. The airplane has been retained for further examination.

Analysis

According to the pilot, during final approach the nose and left main landing gear lights were illuminated green. The red “in-transit” light was illuminated for the right main landing gear. He overflew the runway and airport personnel confirmed that the right main landing gear was partially extended. He unsuccessfully attempted to cycle, manually extend, and use g-force maneuvers to extend the right main landing gear. The pilot performed a landing and kept the right wing off the ground as long as he could during the landing since the right main landing gear was partially extended. During the landing rollout, the right wing impacted the runway, the airplane veered to the right and came to rest off the right side of the runway, resulting in substantial damage to the right aileron. A post-accident examination of the right main landing gear revealed that the oleo strut was under inflated, therefore, the strut did not extend the wheel properly and could not retract correctly into the right main landing gear wheel well. Then, when the landing gear was extended, the wheel became lodged on the wheel well and could not extend properly. After the airplane was lifted off the runway, the right main landing gear was extended by hand and it locked into place. The most recent annual inspection was performed about 5 months before the accident, and at that time the left main landing gear actuator was serviced. The maintenance entry did not record any service to the right main landing gear.

On May 25, 2021, about 1747 eastern daylight time, a BHI H60 Helicopters LLC, UH-60A, N9FH, was destroyed when it was involved in an accident near Leesburg International Airport (LEE), Leesburg, Florida. The pilot, copilot, and two crew members were fatally injured. The helicopter was operated as a Title 14 Code of Federal Regulations Part 91 post-maintenance test flight.
  
  According to the operator, a new water tank and snorkel were installed on the helicopter to facilitate firefighting operations in accordance with supplemental type certificate (STC) #SR00933DE on May 17, 2021. Several days of ground testing and calibration were performed before the accident flight, which was the first flight after the STC was installed.
  
Witnesses reported that the helicopter made six uneventful passes in front of the operator’s hangar at LEE and dropped water that was picked up from a lake adjacent to the airport. On the seventh pass, an employee of the operator noticed the snorkel swinging. He called the LEE air traffic control tower on the phone and told the controller to ask the pilot of the helicopter to slow down and land immediately. Before the controller could contact the pilot, the helicopter transitioned to forward flight, gaining altitude and airspeed. The employee noticed the snorkel “violently” swinging and he then heard a loud bang, which he believed was the result of the snorkel contacting the main rotor blades or tailboom. He then saw pieces, and then the tail section separating from the helicopter. The helicopter started to spin and fell below the tree line. He heard a loud explosion and saw smoke rise above the tree line.
  
According to another witness who worked for the operator, she missed the helicopter’s first pass but watched the remaining six passes. She noted that the water being dropped from the tank was “very dirty.” On the helicopter’s last pass, while it was coming in and slowing down, she noticed the snorkel swing in a large circle and at one point, the snorkel end came very close to the main rotor blades. She immediately started waving her arms at the pilot to try and get his attention, but he did not see her. As the pilot transitioned to forward flight, she ran beside the helicopter and continued to wave her arms. Shortly after the helicopter climbed and gained airspeed, she heard a loud bang and saw multiple main rotor blades separate and hit the tail section. She then saw the tail section fall to the ground and the helicopter enter a flat spin. 

The helicopter was located in heavily wooded, swampy terrain about 1322 ft east of runway 3. The helicopter came to rest slightly inverted on its left side on a heading of 040° magnetic. The tail rotor section was found about 78 ft north of the main wreckage. One half of a rotor blade was found about 600 ft south of the main wreckage and one half of another rotor blade was found about 200 ft west of runway 3. Parts of the newly installed water tank and snorkel assembly were found on the west side edge of runway 3. The water pump housing, which was installed near the snorkel inlet was heavily fragmented. The stainless-steel snorkel suction cage was located about 50 ft west of runway 3 and collocated with a section of main rotor blade. There were several pieces of fairings and lightweight
material lodged in the top of trees along the flight path from the edge of the tree line to the main wreckage.
  
The landing gear, main rotor system, main rotor drive system, engine, hydraulic system, and the forward portion of the tail rotor drive system were thermally damaged by the postcrash fire. The majority of the cockpit, cabin, and flight controls were consumed by the postcrash fire.
  
The wreckage was recovered and retained for further examination.
