On March 6, 2022, at 2359 eastern standard time, a Eurocopter Deutschland GMBH, MBB-BK 117-C-2 helicopter, N29VA, was substantially damaged when it was involved in an accident at the Virginia Highlands Airport (VJI), Abingdon, Virginia. The pilot sustained serious injuries and the two flight paramedics were not injured. The helicopter was operated by the Virginia Department of State Police as a public state aircraft.

The pilot reported that his duty-day started at 0800 and he had completed four air medical transportation flights prior to receiving an additional call at 2200 for a patient transfer from the Lonesome Pine Hospital (VG50), Big Stone Gap, Virginia, to Holston Valley Hospital (3TN5), Kingsport, Tennessee. The flights from his base (VJI) to both hospitals were routine.

During the positioning flight from 3TN5 to VJI, the pilot reported that on liftoff he “slowly pulled up on the collective, the helicopter became light on the skids, and I noticed what sounded like a surge in the engines. I also noticed the [engine power] needles surged upward.” He asked the flight paramedic seated in the front seat whether he heard the noise; the paramedic reported after the accident that he noticed the helicopter skid slip nose right but didn’t think anything was abnormal. The pilot continued the liftoff into a hover, evaluated the engine instruments and noted normal indications. He then proceeded en route without further incident to VJI.

Upon arrival the pilot maneuvered in a sideward right hover to align with a platform dolly that was positioned on the apron. The pilot visually verified that the helicopter was centered over the dolly and lowered the collective to land. He then felt the rear of the skids make contact with the dolly; however, he then heard “what sounded like an engine surge similar to the sound the helicopter had made” on liftoff from 3TN5. Subsequently, the helicopter moved toward nearby hangars, and entered “an extreme nose low position.” The pilot maneuvered the cyclic full aft and left, in an attempt to arrest the descent. He then felt ground contact and “forcefully slammed the collective to the floor.” The helicopter came to rest in a parking lot next to the dolly after skidding through grass and an evacuation was performed after the rotor blades were secured.

Photographs of the helicopter revealed that it came to rest up-right, and the dolly’s orientation shifted from its original north-south position after the helicopter contacted it. Substantial damage was sustained to the fuselage and main rotors. A preliminary visual examination of the helicopter’s engine did not reveal any obvious anomalies. The helicopter was retained for further examination.

On March 7, 2022, about 1045 eastern standard time, a Cessna 528B, N22AU, was substantially damaged when it was involved in an accident at Martin State Airport, (MTN) Baltimore, Maryland. The airline transport pilot and copilot were not injured. The airplane was operating under the provisions of Title 14 Code of Federal Regulations as a Part 135 on-demand charter flight.

According to the pilot, they maintained a stable approach to runway 15, a 6,997-ft-long paved runway, in windy and gusty conditions. They added about 5 knots to their reference speed to account for the added gust factor. Their landing performance data indicated they would need about 3,000 ft of runway to land. Upon landing, the copilot reported that the flare was normal, followed by a “good touchdown” and everything appeared to be going well until the pilot asked, “Why aren’t the brakes working?” The copilot applied brakes on his side and was unsuccessful in achieving any braking action. As they were nearing the departure end of the runway, they attempted to locate and use the emergency brake but were unable to before the airplane departed the paved runway surface.

Preliminary radar and Automatic Dependent Surveillance-Broadcast (ADS-B) data indicated the airplane was established on a final approach on a heading of 136° and lined up with the runway centerline. Throughout the final approach, the altitude, speed, descent rate and heading appeared stable. About 2,400 ft out from the runway threshold, the airplane was at 325 ft with a ground speed of 116 knots before ADS-B data ceased. A review of recorded data from MTN revealed that, at 1053, wind was from 210° at 12 knots gusting to 21 knots.

The airplane exited the end of the runway then proceeded through a marsh before colliding with a security fence and coming to rest. A Federal Aviation Administration inspector confirmed that the fuselage and wings sustained substantial damage.

 The wreckage was retained for further investigation.

A small plane skidded off the runway Monday morning at Martin State Airport in Middle River.

Maryland Aviation Administration officials told 11 News a Cessna 525B CitationJet CJ3 arrived at the airport around 11 a.m. and overran the end of the runway.

No injuries were reported to the two people onboard the aircraft.

On March 8, 2022, at 1417 mountain standard time, a Cessna P210N airplane, N210GE, was destroyed when it was involved in an accident near Englewood, Colorado. The pilot and flight instructor both sustained minor injuries. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

The pilot reported that the flight was cleared for an instrument approach as they neared the intended destination airport. The initial portion of the approach was uneventful. After contacting the control tower, the controller requested that they sidestep to the parallel runway for landing. The runway approach thresholds were not aligned, and the glide path needed to be extended. As the pilot attempted to increase the throttle input, the engine did not respond. He subsequently realized the engine had lost power. The flight instructor noted that there were no indications the engine had lost power – no vibration, coughing, or sputtering. The airplane was unable to glide to the runway, and the flight instructor executed a forced landing to the center median area of an interstate highway.

The accident site was located about one-half mile short of the runway threshold. A postimpact fire consumed portions of the forward fuselage and cabin area. Post-recovery airframe and engine exams are planned.

DOUGLAS COUNTY, Colorado (CBS4) – Two people walked away after a small plane crashed on E-470 near Centennial Airport on Tuesday, March 8.

The highway is currently closed in both directions at Peoria. The two onboard a Cessna P210N Pressurized Centurion were taken to the hospital for evaluation, but South Metro Fire tells CBS4 their injuries are minor.

Firefighters battled a fuel fire near the crash site, but it is now contained. They add the plane was flying from the south, heading north before crashing.

There is no current estimated time the highway will reopen.

On February 20, 2021, about 1700 eastern standard time, a Cessna P210N, N210GE, was substantially damaged when it was involved in an accident at Bartow Executive Airport (BOW), Bartow Florida. The pilot was not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

According to the pilot, earlier on the day of the accident, he flew from Greenville Downtown Airport (GMU), Grenville, South Carolina to BOW to attend a memorial service. While he attended the memorial service, a fixed-base-operator at BOW performed an oil change on the airplane. The pilot subsequently departed BOW for the return flight to GMU. As the airplane climbed through 5,000 ft mean sea level (msl), he heard a “clicking” sound every 1 minute and noted that the sound occurred simultaneously with a “pegging” of the analog oil temperature and oil pressure gauges to the right, and the amp meter to the left (full discharge). He checked the circuit breakers and fuses but did not observe any anomalies.

The airplane was at 14,000 ft msl over Gainesville, Florida when the pilot decided to return to BOW instead of continuing the flight over mountains at night with a possible electrical problem. During the return to BOW the “clicking” sound subsided. The pilot subsequently extended the landing gear, flaps and speed brakes. On short final approach and over the runway numbers, everything was normal. The wind was a crosswind from the left. As the pilot flared, he kept the airplane’s left wing low and touched down on the left main landing gear and then the nose gear, at which time the tail seemed to “fall out” from under him. The airplane leaned to the left and continued in a straight path to a stop. He pulled the mixture control, shut off the master switch and magnetos and egressed. The pilot then walked around the airplane and observed that the nose landing gear was properly extended; however, the two main landing gear had collapsed resulting in damage to the horizontal stabilizer, left wing and fuselage.

The wreckage was recovered and retained for further examination.
