Analysis

The pilot reported that, during a three-point landing, the tailwheel touched down first, and as the main landing gear touched down the airplane bounced. He applied more back pressure on the yoke, and as the airplane settled on the main landing gear it veered to the left. He applied right rudder and brake with no avail. Subsequently, the right wingtip struck the ground and the airplane nosed over.

The airplane sustained substantial damage to the left wing, the right-wing lift strut, and the empennage.

The pilot reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation.

The automated weather observation system located at the accident airport reported, that about the time of the accident, the wind was from 010° at 6 knots. The pilot landed on runway 9.

Probable Cause and Findings

The National Transportation Safety Board determines the probable cause(s) of this accident to be:
The pilot's improper landing flare, which resulted in a loss of directional control and a nose-over.

Findings

Aircraft
Landing flare - Not attained/maintained (Cause)
Directional control - Not attained/maintained (Cause)

Personnel issues
Aircraft control - Pilot (Cause)

Analysis

The pilot reported that, during a biennial flight review, as the tailwheel touched down on landing, the airplane veered to the right. He added that, he was unable to correct quickly enough to prevent the airplane from exiting the runway to the right. The right main landing gear exited the runway, and the airplane ground looped to the right.

The airplane sustained substantial damage to the left aileron.

The pilot reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation.

Accident Number: GAA18CA171

Registration: N969RA

Aircraft Damage: Substantial
Defining Event: Abnormal runway contact
Injuries: 2 None
Flight Conducted Under: Part 91: General Aviation - Instructional

The flight instructor reported that he was providing multi-engine instruction in an airplane with retractable landing gear to a private pilot that held a single engine airplane rating. During the pilot's first simulated engine failure, single engine approach to land, he did not extend the landing gear, and the flight instructor did not verify that the landing gear were extended. During the approach, the landing gear warning horn was heard by both pilots but disregarded because of the nature of the maneuver. The airplane touched down on the runway with the landing gear retracted.

The airplane sustained substantial damage to the lower fuselage.

According to the Airplane Flying Handbook, page 1-6 Use of Checklist: The importance of consistent use of checklists cannot be overstated in pilot training. A major objective in primary flight training is to establish habit patterns that will serve pilots well throughout their entire flying career. The flight instructor must promote a positive attitude toward the use of checklists, and the student pilot must realize its importance.

The pilot reported that there were no mechanical malfunctions or failures with the airplane that would have precluded normal operation.
