Analysis 

The pilot reported that, while flying over the grass runway, it appeared "rough but serviceable." During landing, he found the surface was extremely rough with tall weeds. During the subsequent departure, acceleration was "less than normal" due to the vegetation. When the airplane reached the pilot's planned abort point for the runway, he realized that he had misjudged the abort point. He initially thought his selected abort point was about 800 to 1,000 ft before the end of the runway, but it was about 100 to 200 ft before the end of the runway. The airplane overran the runway, crossed a road, and struck a barbed wire fence.

The airplane sustained substantial damage to the fuselage and left wing.

The pilot reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation.

The Federal Aviation Administration Airport Facility Directory for the accident airport, in part, stated: "[Runways] not recommended for [takeoff or landing]."

The pilot's improper preflight planning that did not identify the unserviceable runway; his subsequent decision to attempt to takeoff from the rough, grassy runway; and his selection of an abort point with insufficient runway remaining, which resulted in a runway overrun.

Analysis 

The pilot of the tailwheel-equipped airplane reported that, during takeoff, the right wing lifted "unexpectedly." He attempted to recover, but the left main landing gear sideloaded and collapsed. The left wing impacted the runway, and the airplane rotated to the left 180° and came to rest on the runway.

The airplane sustained substantial damage to the left wing.

The pilot reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation.

The automated weather observation station located on the airport reported that, about 7 minutes before the accident, the wind was from 320° at 14 knots. The airplane was departing from runway 21.

Analysis 

The solo student pilot of the gyroplane reported that, during takeoff, he pulled the stick back and advanced the throttle to full. He accelerated and noticed "there was no tendency for the nose to rise," so he applied back pressure, and the gyroplane immediately turned left, rolled onto its side, and skidded off the runway to the left.

The gyroplane sustained substantial damage to the main rotor, fuselage, and empennage.

The student reported that there were no preaccident mechanical failures or malfunctions with the gyroplane that would have precluded normal operation.

The flight instructor provided a video of the student pilot's accident takeoff. The video showed the gyroplane accelerating down the runway, rapidly pitching up, and immediately rotating to the left before impacting the runway.

Analysis 

The pilot and a safety pilot were practicing instrument maneuvers, approaches, and landings at different airports during the flight. The airplane entered the traffic pattern for a practice landing at an airport. According to the safety pilot, the airplane was high and slow during the final approach. About 10 ft above ground level, the airplane "floated a little longer," and the pilot initiated a go-around by applying full throttle. During the attempted go-around, both occupants applied conflicting "correction" inputs to the control yokes; the pilot applied back pressure, and the safety pilot applied forward pressure. The airplane stalled, the left wing impacted terrain, and the airplane came to rest upright adjacent to the runway.

The airplane sustained substantial damage to the forward fuselage and both wings.

Both pilots reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation.

The pilot statements were conflicting regarding the accident details and sequence of events. Based on the statements and airplane damage, it is likely that the conflicting control inputs resulted in the pilots' failure to maintain a proper airspeed and that the airplane entered an aerodynamic stall during the attempted go-around. Neither pilot reported that verbal communication was established during the landing sequence.

The pilot's and safety pilot's conflicting control inputs during landing, which resulted in their failure to maintain a proper airspeed and resulted in an aerodynamic stall. Analysis 

The pilot reported that, during landing, the airplane bounced multiple times, so he added power to go around, but the left wing dropped, and the airplane "veered" left and dropped to the ground. The airplane touched down in the grass to the left of the runway, the nose landing gear collapsed, and the airplane nosed over.

The airplane sustained substantial damage to the fuselage and empennage.
