On February 27, 2022, about 0958 eastern standard time, an experimental, amateur-built Vans RV-12IS, N123RR, was destroyed when it was involved in an accident in Florida Bay, about 20 miles northwest of Big Pine Key, Florida. The sport pilot and passenger have not been located. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

The airplane was reported overdue to the Federal Aviation Administration on February 28, 2022, after the pilot did not report to work. The rudder and right horizontal stabilizer were located by the U.S. Coast Guard on March 4, 2022. As of the publication of this report, no other sections of the airplane have been located; however, the Florida Fish and Wildlife Conservation Commission planned an additional search. The pilot was not in radio contact with air traffic control at the time of the accident. Automatic Dependent Surveillance- Broadcast (ADS-B) data revealed that the airplane was in the vicinity of moderate precipitation at the end of the data.

According to a friend, the pilot usually received weather briefings via a ForeFlight app on his phone.

MONROE COUNTY, Florida – The U.S. Coast Guard confirmed Friday that it has suspended its search for a couple who is believed to be missing at sea after their plane crashed sometime Sunday.

The search was suspended on Thursday.

Coast Guard officials said divers with the Monroe County Sheriff’s Office recovered two pieces of a plane Thursday, identifying the tail number of the missing single-engine plane on the ocean floor near Big Pine Key.

Officials believe the Vans RV-12 plane Alexandra “Ali” Tufo and her boyfriend Thomas “Tommy” Campana were flying from Fort Lauderdale to Key West Sunday crashed about 19 miles north of Big Pine Key.

“Our thoughts and prayers go out to the family and friends of Alexandra Tufo and Thomas Campana,” said Cmdr. Lindsey Seniuk, search and rescue mission coordinator, Coast Guard District Seven. “This was a challenging case and it made the decision to suspend that much harder after our partners found the debris field. The MCSO divers are the experts in this new phase of the search, and our crews will be ready if our assistance is requested.”

Coast Guard crews began searching for the overdue plane on Tuesday after they were alerted by the Federal Aviation Administration,

It’s unclear if Campana submitted a flight plan. He also did not have a FlightAware tracking system, which experts say isn’t uncommon for small single-engine planes.

In a statement, the FAA said: “The FAA does not require private aircraft operators to file flight plans, but recommends they do.”

“I don’t know why someone would do that, especially if you’re trying to have safety mechanisms in place,” Tufo’s sister, Lana Tufo, said. “She told me Tommy was an amateur pilot and he had gotten his license, and they had been going up into the air and she was really scared to go up in a small plane. But she was so happy she overcame her fear.”




NEAR BIG PINE KEY, Florida (WSVN) — Officials have suspended their search for a plane that disappeared near the Florida Keys.

Investigators said the aircraft had a couple onboard and took off Sunday from Palm Beach headed to Key West.

The sister of Alexandra Tufo, one of the people on the flight, told 7News that Coast Guard divers found the plane’s tail and wing in the open ocean near the original search area, 15 miles north of Big Pine Key.

Tufo and her boyfriend have not been found.

The Federal Aviation Administration first alerted local authorities on Tuesday that the single-engine plane went missing.

According to flight records, Tufo’s boyfriend received his pilot’s license in January.

Due to weather conditions, the recovery mission will continue on Tuesday.

On March 6, 2022, about 1015 eastern standard time, a Beech A35, N764B, was substantially damaged when it was involved in an accident near Waco, Georgia. The commercial pilot and passenger were not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

The pilot reported that he was on a short cross-country flight when the engine began to lose power. He observed that the fuel pressure dropped to zero and attempted to regain power by hand-pumping the auxiliary fuel pump. He was unable to regain power and contacted air traffic control to request radar vectors to the nearest airport. As the airplane continued to lose altitude, the pilot conducted an emergency landing on a road. During the emergency landing, the airplane collided with power lines before coming to a stop on the road. After landing, the pilot exited the airplane and discovered a leaking fuel line within the engine compartment.

Examination of the wreckage by a Federal Aviation Administration inspector revealed structural damage to the vertical stabilizers and fuselage.

The wreckage was recovered for further examination.

Analysis

The flight instructor and student pilot were completing stop-and-go landings at night at an uncontrolled airport. During the second landing, a deer collided with the left rear fuselage and the empennage, then the airplane yawed left. The flight instructor took control of the airplane and taxied off the runway without further incident. The airplane sustained substantial damage to the left horizontal stabilizer.

On March 5, 2022, about 1200 central daylight time, a Piper PA-28-235, N5078M, was substantially damaged when it was involved in an accident near Monee, Illinois. The pilot sustained minor injuries. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

The pilot stated that the flight was to remain in the departure airport traffic pattern. During a climb after takeoff, the engine lost power. He said that he checked the throttle, mixture, and the magnetos. He then made a 180° turn to return to the departure airport and during the approach to the airport, he determined that the airplane was unable to attain the airport. He landed the airplane on a field, and on impact the airplane sustained substantial damage to the motor-mount.

Postaccident examination of the airplane revealed that the left tip fuel tank contained about 23 ounces of fuel, the left main fuel tank level was about 4 inches, the right tip fuel tank contained no useable fuel, and the right main fuel tank contained about 1 inch of fuel. 

The airplane battery switch was turned on, and the fuel gauge indications were: left tip tank – 0 gallons, the left main tank – 5 gallons, the right tip tank – 0 gallons, the right main tank – 0 gallons.

The carburetor was disassembled, and the carburetor bowl contained about 0.4 inch of fuel. 

There was no fuel in the fuel line leading to the carburetor.

There were no mechanical anomalies that would have precluded normal engine operation.

Analysis

The pilot reported that he encountered a very strong gust of wind during landing rollout, which pushed the airplane off the runway into a ditch. Subsequently, the airplane nosed over and sustained substantial damage to the right wing and vertical stabilizer. The pilot reported that the airplane had no preimpact mechanical malfunctions or failures that would have precluded normal operation.

PERRY COUNTY, Kentucky (WYMT) - It was a close call at Wendell H. Ford Airport in Perry County on Saturday as high winds turned a routine landing into an alarming incident.

The incident happened at around 3:45 p.m. when gusts of wind blew the pilot’s Cessna 182 off of the runway, flipping the plane upside down before it landed on top of a small cliff nearby.

Both the pilot and passenger in the aircraft were not injured.

Officials are not releasing their identities at this time.

Franklin Parish Sheriff's Office -

Sheriff Kevin Cobb reports that at approximately 1012 hrs the Franklin Parish Sheriff's Office received a 911 call reporting an airplane crash at the Winnsboro Municipal Airport. 

Two male occupants were transported to the Franklin Medical Center for medical treatment.

At this time the crash is under investigation by the Franklin Parish Sheriff's Office and has been reported to the National Transportation Safety Board for further investigation. 

Sheriff Cobb stated, "I am thankful both occupants were communicating to us after the accident and that those in the residential area were not impacted."

Analysis

The pilot reported that the tailwheel-equipped airplane drifted right during the landing rollout in crosswind conditions. The pilot applied left rudder to correct the deviation; however, the right main landing gear contacted a snowdrift on the runway edge and the right wing impacted a snowbank. The airplane then yawed right, departed the runway, nosed over, and came to rest inverted, which resulted in substantial damage to the fuselage and rudder. The pilot reported that the airplane had no preimpact mechanical malfunctions or failures that would have precluded normal operation.

On March 10 2022, about 1430 Alaska daylight time, a Cessna 310R twin-engine airplane, N3UR, was substantially damaged when it was involved in an accident near McGrath, Alaska. The pilot and three passengers were not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

According to the pilot, he back taxied along runway 16 towards the departure end of the runway. When he arrived at the end of the runway, he started to turn to the airplane to align it with the reciprocal runway. However, as he began the turn, the right main landing gear collapsed and the wing and propeller both contacted the runway. The accident pilot, who was also a certificated aircraft mechanic, stated that he completed a preliminary inspection of the right main landing gear, which revealed that the retraction lug on the gear trunnion had separated and a bolt had sheared, which allowed the gear lock down arm to fold and the landing gear to retract inward on the landing gear door.

Photographs provided by the pilot showed substantial damage to the elevator.

The wreckage was retained for further examination.

Analysis

According to the pilot, the purpose of the flight was to practice wheel landings in the tailwheel equipped airplane. PLAINVILLE, Connecticut — A close call Thursday morning at Plainville's Robertson Airport.

Just after 11:30 a.m., a single-engine plane flipped over nose first after landing at Robertson Airport. The pilot, the only person in the plane, escaped serious injury.

"It appears that the single-engine plane, after landing, veered off to the right-hand side (of the runway) onto a grassy area and then flipped over onto its roof," said Chief Chris Vanghele of the Plainville Police Department.

But why remains a question Vanghele could not answer.

"It could be something with the brakes. We don't know," he said.

The lone occupant, according to sources, was 81-year-old pilot Frank Dworak.

"He was transported to the hospital, which seems right now to be a very minor injuries," Vanghele added.

And he noted that it does not appear the pilot suffered a medical event.

"He was conscious and he was alert and talking while he was being extricated from the plane," the Chief said.

The FAA has not responded to FOX61's request for the pilot's flight plan and Robertson Airport officials declined to comment when asked if the plane is based at Robertson.

"The plane itself was made safe by the fire department and mechanics here to ensure that there is nothing flammable and no issues during the investigation," Vanghele said. "The skid marks in the grass are probably only about 50 to 60 feet long and then at you know when it dove nose-first the propeller hit and then the plane flipped over onto its roof."

The airport was closed to air traffic for several hours. The plane was carted away by investigators.
