Location: Bay City, Wisconsin
Accident Number: ERA22FA399
Date and Time: September 6, 2022, 13:25 Local
Registration: N11HC
Aircraft: CONWAY PHILIP J Glasair Super II SFT
Injuries: 2 Fatal
Flight Conducted Under: Part 91: General aviation - Instructional

On September 6, 2022, at 1325 central daylight time, an experimental, amateur-built Glasair Super II SFT airplane, N11HC, was destroyed when it was involved in an accident in Bay City, Wisconsin. The flight instructor and a commercial pilot-under-instruction were fatally injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 instructional flight.

The flight departed from Rochester Regional Airport (RST), Rochester, Minnesota about 1218. Following some air work, the flight proceeded to Red Wing Airport (RGK), Bay City, Wisconsin and entered left traffic for the visual airport traffic pattern to runway 9. According to Automatic Dependent Surveillance-Broadcast (ADS-B) data, three circuits of the traffic pattern were flown (it is not known if the airplane landed during the approaches) and a fourth traffic pattern was initiated. The ADS-B data indicated that the airplane turned onto the base leg of the airport traffic pattern about 1 mile sooner than on the previous approaches. A witness reported that the airplane, while on base leg, banked sharply to the left and pointed nose down before it crashed. The airplane impacted the ground about 1/3 nautical mile west-northwest of runway 9.

An examination of the accident site revealed that the airplane impacted terrain on a heading of about 045° and the main wreckage came to rest about 80 ft from the point of initial impact. There was no fire. All structural components of the airplane were located within the confines of the wreckage path. Both wing fuel tanks and a header tank were breached during the impact, and no fuel remained inside the tanks. The owner reported that he filled the tanks to their 50-gallon capacity before the flight.

The owner, who was the airplane builder, reported that the purpose of the flight was build time requirements for insurance purposes so that he could include the commercial pilot on his policy. This was the first flight to satisfy those requirements and the two accident pilots had not flown together previously. The flight instructor flew with the owner on September 1, 2022, and the owner stated that the flight instructor performed all tasks satisfactorily, including air work, stalls, patterns, and landings. Other than the 2-hour flight on September 1, the flight instructor had no previous experience in the Glasair. The commercial pilot had flown with the owner as a passenger; however, he had no logged time in the Glasair.

The airplane was equipped with a Garmin G3X glass cockpit navigation system. The non-volatile memory card from the flight display recorded data from the entire accident flight, including engine and systems performance. The memory card was retained for further investigation.

Fairfield Police Department - Essex County, New Jersey -

Fairfield, New Jersey - On September 9, 2022, at approximately at 11:32 AM, the Fairfield Police Department was notified of an airplane crash at the Essex County Airport located at 125 Passaic Avenue. Police, fire, and EMS was immediately dispatched to the scene. First arriving officers found a Piper PA-39-160 Twin Comanche plane on the grass just off of Runway 04.

The plane was occupied by the pilot, an 84-year-old male from Kew Gardens, New York, and a passenger, an 83-year-old female from Dix Hills, New York. Neither claimed to be injured as a result of the crash. Preliminary investigation revealed that the plane had initially attempted to take off, but this take off was aborted when it was found that the passenger’s door was not properly closed. After this was corrected, the plane taxied and then attempted to take off again, but the pilot apparently aborted this take off which resulted in the plane leaving the runway. It is still unclear as to why the pilot aborted this take off. He was transported to a local hospital for evaluation.

The Federal Aviation Administration was contact and responding to assume the investigation.

A single-engine airplane lost power near Galion Municipal Airport Friday night in Richland County and crash-landed in a soybean field off Airport Road.

The pilot James McKee, 72, of Caledonia in Marion County, suffered only very minor injuries, according to Sgt. Mike Smith of the Mansfield post of the Ohio Highway Patrol, who was on the scene of the crash. The incident occurred around 6 p.m.

The pilot declined comment, saying only that he had lost power. The tail of the plane fell off when the plane struck a utility pole and was on the ground nearby.

At the scene, Ronks Towing brought two large tow trucks and a crane carefully lift the copper-colored plane onto a trailer as the pilot watched.

Troopers at the scene said the pilot told them he took off and made a loop around Galion Airport when he lost power.

Phillip Billow, whose house sits nearby on Airport Road, was outside watching the tow truck operators gently lift the plane as troopers contacted the Federal Aviation Administration by cell phone.

The road is secluded and nestled by cornfields on each side and has very little motor vehicle traffic.

Ronk's employee Derrick Hamilton said he was able to photograph the plane which landed in the soybean field. And members of the tow truck crew pulled the plane onto the paved road.

He said they don't get too many calls for towing airplanes, adding he was glad the pilot was OK.

LINCOLN COUNTY, Missouri — One man was injured when a small kit airplane crashed Wednesday afternoon in unincorporated Lincoln County, Missouri, authorities said.

Cpl. Lance Bolton, a spokesman with the Missouri State Highway Patrol, said the plane clipped power lines at about 3:30 p.m. along Wilson Road near North Highway 61. The crash site is about an hour northwest of St. Louis.

The pilot, a man in his 60s, was transported by ambulance to a nearby hospital with "moderate injuries." The other man, in his early 70s, refused treatment, Bolton said.

A spokesman for the Federal Aviation Administration said the plane that crashed was a single-engine Zenith 750. It was manufactured in 2011 by Zenith Aircraft Co., a Mexico, Missouri-based company, according to FAA records.

FAA records also show the plane is registered to Johnny Hermann. Hermann owns Herrmann-Faulk Airfield, a private runway near where the plane crashed. It's unclear if Hermann was one of the two men on board.

The FAA and the National Transportation Safety Board are investigating what led to the crash.

A Learjet 36 landed on a beach retaining wall Friday after sliding off the end of a runway at the Naval Air Station North Island in Coronado, ending up with its nose in the bay, the FAA confirmed.

The incident was reported shortly after 1 p.m. The Naval Air Station confirmed the aircraft was of a civilian contractor. Two people were on board, but they did not suffer serious injuries, Navy officials said.

Both were still transported to the hospital as a precaution, Navy officials added.

The FAA and the National Transportation Safety Board are investigating the incident. According to FlightAware, the Learjet 36 had taken off at 12:22 p.m. before returning to North Island, lasting a total of 55 minutes in the air before the incident.

"I looked over and there was a massive amount of firetrucks and I could clearly see it was a plane that was partially in the water," said one witness.

“I was happy that it didn't become like worse than it was just because there are like a lot of businesses, and like things right near everything," said Jasmine Urquidez, an employee at a nearby restaurant.

NBC 7 was able to confirm that the registered owner of the Learjet 36 is GH Equipment LLC.

The crash occurred as Tropical Storm Kay brought heavy winds and buckets of rain to the county. A flood watch and warning and high wind warning were in place for some areas of the county Friday.

Although, it is unknown if the crash is weather related.
