On January 13, 2022, about 1306 mountain standard time, an Extra Flugzeugbau GMBH, EA-300L airplane, N203EX, was substantially damaged when it was involved in an accident at the Phoenix-Mesa Gateway Airport, Phoenix, Arizona. The pilot and passenger were not injured. The airplane was operated as a Title 14 Code of Federal Regulations (CFR) Part 91 instructional flight.
The pilot reported that the right wheel brake failed during the landing roll and the airplane subsequently ground looped. During the accident sequence, the right gear collapsed, and the right aileron was substantially damaged. The pilot stated the preflight, taxi, takeoff, and flight was uneventful until the landing roll.
Post-accident examination of the brake assembly by a Federal Aviation Administration inspector revealed that the brake pad had separated from the brake assembly. A brake pad was found on the airplane’s taxi route and was recovered for future examination.

ReplyDeleteI had a mechanic that screwed up an annual this way once. When taxing out for post annual flight, I got a radio call on the CTAF to return to base. When I returned, the AP said that he found a brake shoe out on the taxi/ramp. I didn't see it during preflight, so it must have come off while taxing. He put it back on and I continued my check flight - and I fired him when I got back.
ReplyDeleteWould think be able to feel that when testing your brakes. I had a friend pick up his car at a repair shop they had left the brake pads out. He backed up went to stop ran right into another car behind him sticking out of the garage bay. I went to pick him up we could see brake fluid the calipers pistons had popped out of the calipers leaked on the ground. They paid for everything to get fixed at the dealer.
