It pointed out poor maintenance, record keeping, a history of failed fuel transfer pumps, what appeared to be a relatively minor fuel leak and poor pilot training but didn't isolate the exact cause. Feel bad for the pilot, he likely didn't know all the details of this poorly maintained aircraft. <br /><br />But at the same time, he should have landed at Dayton or one of the other nearby airports when the problem arose instead of trying to make an airport 30 miles away.
