Aircraft: BELLANCA 8KCAB, registration: N55067
Injuries: 1 Uninjured.
NTSB investigators used data provided by various entities, including, but not limited to, the Federal Aviation Administration and/or the operator and did not travel in support of this investigation to prepare this aircraft accident report.
The pilot of a tailwheel-equipped airplane reported that, during the takeoff roll, as the airplane crested a hill about a third of the way down the grass runway, the airplane was close to the right edge of the runway. He added that he could not clearly distinguish the runway edges and applied left rudder, but in doing so, the airplane departed the runway surface to the left and nosed over in rough muddy terrain.
The left wing sustained substantial damage.
The pilot reported that there were no preaccident mechanical malfunctions or failures with the airplane that would have precluded normal operation.
The National Transportation Safety Board determines the probable cause(s) of this accident as follows:
The pilot's failure to maintain directional control during the takeoff roll.

The Kansas Highway Patrol reported a Bellanca 8KCAB Decathlon piloted by Milton D. Pinkston, 86, St. John, aligned the takeoff path incorrectly.
The aircraft went too far to the east, traveled off the runway and entered a muddy wheat field. The landing gear dug in and the aircraft flipped.
Pinkston was not injured.

DESCRIPTION OF DAMAGES: Propeller; 1 blade is non-repairable; OH required, Engine sudden stoppage requires inspection; Accessory inspections; Firewall is bent at lower engine mount attach points;Fuselage is twisted (door misaligned, wings misaligned, cowling misaligned); Fuselage tubular structure is bent; Upper and lower cowling are damaged; Right wing attach point is bent; Right wing forward spar appears stressed; Right wing lift strut and right wing fuel tank punched through the wing; Left wing spar appears stressed; Left wing attach point is bent; Left wing tip is destroyed; Tail (rudder/vertical fin) impacted.
